【摘要】：目的分析阻塞性睡眠呼吸暫停低通氣綜合征(OSAHS)血栓前狀態(tài)的危險因素,為OSAHS患者靜脈血栓栓塞癥的預防提供參考和借鑒。方法選取2014年6月至2016年7月在新疆醫(yī)科大學附屬第一醫(yī)院經多導睡眠監(jiān)測(PSG)明確診斷為OSHAS并進行了凝血、血栓與纖溶檢測及炎癥因子組合測定,同時排除存在其他可能影響凝血功能因素的患者共238例。其中符合血栓前狀態(tài)參考標準的患者56例(即血栓前狀態(tài)組),剩余182例患者凝血及血栓與纖溶檢測不滿足參考標準,從中隨機抽取59例患者為對照組(即非血栓前狀態(tài)組)。比較兩組患者年齡、性別、體重指數(BMI)、睡眠呼吸暫停低通氣指數(AHI)、白細胞介素-6(IL-6)、合并慢性阻塞性肺疾病(簡稱慢阻肺)和合并高血壓的狀況。結果非條件Logistic回歸分析顯示OSAHS血栓前狀態(tài)的危險因素有年齡(OR=1.202,95%CI:1.107~1.305)、IL-6(OR=1.127,95%CI:1.014~1.252)、AHI(OR=1.151,95%CI:1.055~1.256)、合并慢阻肺(OR=4.749,95%CI:1.046~21.555)。結論年齡、AHI、IL-6、合并慢阻肺可能是OSAHS患者血栓前狀態(tài)的危險因素,其中合并慢阻肺可能是最重要的危險因素。
[Abstract]:Objective to analyze the risk factors of prethrombotic state of obstructive sleep apnea hypopnea syndrome (OSAHS) and to provide reference for the prevention of venous thromboembolism in patients with OSAHS.Methods from June 2014 to July 2016, OSHAS was diagnosed by polysomnography in the first affiliated Hospital of Xinjiang Medical University, and coagulation, thrombus and fibrinolysis were detected and inflammatory factors were determined.A total of 238 patients were excluded from other possible factors affecting coagulation function.Among them, 56 patients (i.e. prethrombotic group) met the reference standard of prethrombotic state, while the remaining 182 patients did not meet the reference standard for blood coagulation and thrombus and fibrinolysis. 59 patients were randomly selected as control group (that is, non-prethrombotic state group).Age, sex, body mass index (BMI), sleep apnea hypopnea index (AHI), interleukin-6 (IL-6), chronic obstructive pulmonary disease (COPD) and hypertension were compared between the two groups.Conclusion AHI IL 6 and COPD may be the risk factors of prethrombotic status in patients with OSAHS, among which COPD may be the most important risk factor.
