【摘要】：目的探討老年性聽力損失患者的言語識別特點。方法以59例老年性聽力損失患者(老年聽損組)為研究對象,年齡61~84歲,平均71.3±6.7歲,其中男23人,女36人,根據(jù)較好耳0.5、1、2、4kHz平均氣導聽閾(pure tone average,PTA)分為輕度聽力損失組(10例)、中度聽力損失組(35例)、重度以上聽力損失組(14例);以11例聽力正常老年人作為對照組;以普通話言語測聽材料中的9張雙音節(jié)詞表分別測試各組的言語識別閾(speech recognition threshold,SRT)和言語識別率,并建立識別-強度函數(shù)曲線(P-I函數(shù)曲線),比較各組的結果。結果老年聽損組PTA(51.65±11.98dB HL)、SRT(50.98±16.05dB HL)明顯高于對照組(PTA 19.55±4.55dB HL、SRT 18.79±7.45dB HL),老年聽損組P-I函數(shù)曲線平均斜率(2.63%±1.59%/dB)低于對照組(4.65%±1.46%/dB)(P0.01);老年聽損男性組SRT值(56.54±17.23dB HL)高于女性(47.99±15.63dB HL)(P0.05);三組不同程度老年聽損組的PTA和SRT明顯高于對照組,隨著聽力損失程度的加重,PTA、SRT閾值明顯增高,組間比較差異有統(tǒng)計學意義(P0.01);不同程度聽損各組的P-I函數(shù)曲線斜率(輕度組2.47%±1.59%/dB,中度組2.76%±1.59%/dB,重度以上組2.42%±1.69%/dB)明顯低于對照組(4.65%±1.46%/dB)(P0.01),不同程度聽損組間P-I函數(shù)曲線斜率比較差異無統(tǒng)計學意義(P0.05)。結論老年性聽力損失患者言語識別閾升高,且男性高于女性,P-I函數(shù)曲線平均斜率下降,曲線右移呈平緩型;隨著聽力損失程度的加重,其言語識別閾升高更顯著。
[Abstract]:Objective to investigate the speech recognition characteristics of senile hearing loss patients. Methods 59 elderly hearing loss patients (elderly hearing loss group) were studied. The average age was 71. 3 鹵6. 7 years old, including 23 males and 36 females. According to the average air-guided hearing threshold of 0.5kHz and 2kHz, pure tone averagePTAs were divided into mild hearing loss group (n = 10), moderate hearing loss group (n = 35) and severe hearing loss group (n = 14). The speech recognition threshold and speech recognition rate of each group were measured with 9 dicyllabic lexical lists in Putonghua speech audiometry. The P-I function curve of recognition intensity function curve was established and the results of each group were compared. Results the average slope of P-I function curve in the elderly hearing loss group was significantly higher than that in the control group (P < 4.65% 鹵1.46% 鹵1.46%), and the average slope of P-I function curve in the elderly hearing loss group was lower than that in the control group (4.65% 鹵1.46% vs 1.46% P 0.01), which was significantly higher than that in the control group (19.55 鹵4.55dB HLN SRT 18.79 鹵7.45dB HLN). The average slope of P-I function curve in the elderly hearing loss group was 2.63% 鹵1.59% 鹵1.59%. The SRT value of the male group with hearing impairment was 56.54 鹵17.23dB HL, the PTA and SRT of the three groups were significantly higher than that of the control group, and that of the male group was higher than that of the female group (47.99 鹵15.63dB HLL P 0.05), and that of the three groups was significantly higher than that of the control group. With the increase of hearing loss, the SRT threshold of PTAT increased significantly. The slope of P-I function curve (2.47% 鹵1.59% dB in mild group, 2.76% 鹵1.59% in moderate group, 2.42% 鹵1.69% in severe group) was significantly lower than that in control group (4.65% 鹵1.46% P 0.01U). Conclusion the speech recognition threshold of the aged patients with hearing loss is higher than that of the controls. The mean slope of P-I curve of male was lower than that of female, and the curve shifted to the right of flat type, and with the increase of hearing loss, the threshold of speech recognition increased more significantly.
