【摘要】：目的探討超聲乳化白內(nèi)障吸除術(shù)中晶狀體后囊膜破裂的相關(guān)風(fēng)險(xiǎn)因素及預(yù)后。方法選取2014年1月-2015月10日于河北省眼科醫(yī)院行超聲乳化白內(nèi)障吸除術(shù)的患者2 409例(2 576只眼)為研究對象進(jìn)行回顧性資料分析,發(fā)生后囊膜破裂的24例(26只眼)患者作為觀察組,以1∶5的比例,從剩余患者中采用隨機(jī)數(shù)列選取120例(135只眼)患者作為對照組,若選中的患者發(fā)生后囊膜破裂,則剔除繼續(xù)選擇。比較兩組患者年齡、性別、術(shù)前眼壓、白內(nèi)障類型及分級、術(shù)前術(shù)后視力、青光眼病史、合并疾病等臨床資料,分析后囊膜破裂的相關(guān)風(fēng)險(xiǎn)因素及預(yù)后。結(jié)果男性晶狀體后囊膜破裂發(fā)生率明顯高于女性(P0.05);術(shù)前視力20/200是后囊膜破裂的相關(guān)風(fēng)險(xiǎn)因素(P0.05);白內(nèi)障Ⅳ級核及以上的后囊膜破裂發(fā)生率明顯增高,差異有統(tǒng)計(jì)學(xué)意義(P0.05);觀察組術(shù)前平均視力(2.3±0.8)LogMAR,術(shù)后平均視力(1.1±0.6)LogMAR,差異有統(tǒng)計(jì)學(xué)意義(P0.05),對照組術(shù)前平均視力(1.7±0.4)LogMAR,術(shù)后平均視力(0.8±0.5)LogMAR,差異有統(tǒng)計(jì)學(xué)意義(P0.05);患者年齡、眼別、術(shù)前眼壓、青光眼病史、合并疾病等與后囊膜破裂無明顯相關(guān)性(P0.05)。結(jié)論性別、術(shù)前視力、白內(nèi)障核分級均影響超聲乳化白內(nèi)障吸除術(shù)患者后囊膜破裂的發(fā)生,臨床診療過程中應(yīng)避免術(shù)后晶狀體后囊膜破裂,以促進(jìn)患者視力恢復(fù)。
[Abstract]:Objective to investigate the risk factors and prognosis of posterior capsular rupture during phacoemulsification. Methods 2 409 patients undergoing phacoemulsification cataract extraction from January 2014 to 10th 2015 were selected. 2 576 eyes were analyzed retrospectively. 24 patients with posterior capsule rupture (26 eyes) were used as observation group, 120 patients (135 eyes) were randomly selected from the remaining patients as the control group, if the selected patients had posterior capsule rupture, The age, sex, preoperative intraocular pressure, cataract type and classification, preoperative and postoperative visual acuity, history of glaucoma, complicated diseases and other clinical data were compared between the two groups.
