DR與MR全脊柱成像技術(shù)在退行性脊柱病診斷中的效果對比

【摘要】:目的評估全脊柱數(shù)字化成像技術(shù)(DR)與MR成像質(zhì)量,分析全脊柱站立位DR及仰臥位MR成像的測量的矢狀面參數(shù)的差異及相關(guān)性。方法搜集2010年11月~2016年3月到我院就診并診斷為退行性脊柱病的61名患者資料,年齡49.9±17.6歲,均行全脊柱站立位DR與仰臥位MR檢查,分析全脊柱拼接的圖像質(zhì)量,在全脊柱DR側(cè)位片、全脊柱MR正中矢狀位圖像測量頸椎前凸角(CL)、胸椎后凸角(TK)、腰椎前凸角(LL)、骶椎傾斜角(SS)、脊柱矢狀位垂直軸(SVA)等參數(shù),并利用配對T檢驗對比DR與MR測量值,并進(jìn)行Pearson相關(guān)性分析。結(jié)果全脊柱DR與MR圖像拼接質(zhì)量優(yōu)良率為100%,全脊柱DR及MR成像上測量CL、TK、LL、SS、SVA值分別為28.37±10.91o、29.98±8.96o、45.61±12.46o、34.38±9.05o、17.20±26.39 mm,及24.34±9.01o、21.22±8.13o、41.45±12.17o、37.45±8.19o、36.51±12.44 mm,DR與MR測量CL、TK、LL、SS、SVA值存在統(tǒng)計學(xué)差異(P值分別為0.000、0.000、0.000、0.001、0.007),相關(guān)系數(shù)r分別為0.69、0.68、0.72、0.51(P值分別為0.000、0.000、0.000、0.000、0.034)。結(jié)論站立位DR與仰臥位MR全脊柱成像圖像質(zhì)量良好,仰臥位MR圖像測量的CL,TK,LL,SS,SVA等脊柱矢狀面平衡參數(shù)與站立位DR相關(guān),但存在明顯區(qū)別,并不能代替站立位DR檢查,須聯(lián)合檢查才能全面評估退行性脊柱病改變。
