【摘要】：目的探討高分辨螺旋CT多平面重建(MPR)和曲面重建(CPR)在亨特綜合征、貝爾面癱中顯示面神經(jīng)管的價(jià)值,為診斷及治療提供可靠的信息。方法回顧性分析北京電力醫(yī)院耳鼻喉科2014年5月到2015年5月105例亨特綜合征(n=42)和貝爾面癱(n=63)患者的臨床及顳骨CT影像學(xué)資料,采用多平面重建(MPR)和曲面重建(CPR)兩種圖像后處理技術(shù)對(duì)面神經(jīng)管進(jìn)行重建,并于相應(yīng)圖像行面神經(jīng)各段的測(cè)量,分析兩種面癱患者面神經(jīng)各段的差異及兩種面癱各項(xiàng)指標(biāo)與時(shí)間的相關(guān)性。結(jié)果 1.兩種周圍性面癱面神經(jīng)管患-健側(cè)比較,差異均有統(tǒng)計(jì)學(xué)意義。2.兩種面癱的相互比較中,亨特綜合征面癱組膝節(jié)直徑、膝節(jié)面積及迷路段直徑較貝爾面癱組更大,差異有統(tǒng)計(jì)學(xué)意義。3.在研究范圍內(nèi),貝爾面癱患者Sunnybrook評(píng)分和乳突段直徑測(cè)量值都與發(fā)病時(shí)間顯著相關(guān),亨特綜合征患者膝節(jié)橫徑、膝節(jié)垂直經(jīng)、膝節(jié)橫截面積均與發(fā)病時(shí)間顯著相關(guān)。結(jié)論面神經(jīng)管高分辨螺旋CT圖像的多平面和曲面重建為兩種周圍性面癱疾病發(fā)展的研究及鑒別診斷提供有價(jià)值的信息。
[Abstract]:Objective To study the value of multi-plane reconstruction (MPR) and surface reconstruction (CPR) of high-resolution spiral CT in the treatment of Hunt's syndrome and Bell facial paralysis, and to provide reliable information for diagnosis and treatment. Methods The clinical and CT imaging data of the patients with Hunt's syndrome (n = 42) and Bell facial paralysis (n = 63) from May 2014 to May 2015 were analyzed retrospectively. The facial nerve tube was reconstructed by multi-plane reconstruction (MPR) and surface reconstruction (CPR), and the correlation between the two facial paralysis indexes and the time was analyzed. Results 1. There was a statistically significant difference between the two types of peripheral facial nerve tubes-healthy side. In the comparison of the two facial paresis, the diameter of the knee joint, the area of the knee joint and the diameter of the lost segment were greater than that of the Bell facial paralysis group, and the difference was statistically significant. In the study, the scores of Sunnybrook and the diameter of the mastoid segment were significantly correlated with the time of onset, and the cross-sectional area of the knee joint and the cross-sectional area of the knee joint were significantly correlated with the onset time. Conclusion The multi-plane and curved surface reconstruction of the high-resolution spiral CT image of the facial nerve tube provides valuable information for the research and differential diagnosis of the development of two peripheral facial paralysis diseases.
