【摘要】：目的探討大面積腦梗死患者合并心力衰竭及糖尿病的臨床治療效果。方法回顧性分析我院收治的15例大面積腦梗死合并心力衰竭及糖尿病患者的臨床資料,觀察其治療過程及結(jié)果,總結(jié)治療解決方案。結(jié)果 15例患者中,療效顯著12例,神經(jīng)功能恢復(fù)比較理想,心力衰竭臨床癥狀及體征基本改善,血糖控制良好;植物狀態(tài)1例(治療過程中出現(xiàn)新發(fā)梗死部位嚴(yán)重,現(xiàn)仍在院治療);死亡2例(心衰加重,臥床時間長,出現(xiàn)多種糖尿病并發(fā)癥)。結(jié)論大面積腦梗死本身致殘率高,合并心力衰竭的糖尿病的病死率、致殘率更高。治療過程中,以控制腦水腫為主,同時注重心衰的處理及血糖的調(diào)整,配合適宜肢體功能鍛煉及合理飲食。最大限度減少患者各種并發(fā)癥的發(fā)生,提高患者的生存質(zhì)量,減輕家庭及社會的負(fù)擔(dān)。
[Abstract]:Objective to investigate the clinical effect of heart failure and diabetes mellitus in patients with large area cerebral infarction. Methods the clinical data of 15 patients with large area cerebral infarction complicated with heart failure and diabetes mellitus treated in our hospital were analyzed retrospectively. the treatment process and results were observed and the treatment solutions were summarized. Results among the 15 patients, 12 cases were effective, the recovery of nerve function was satisfactory, the clinical symptoms and signs of heart failure were basically improved, and the blood glucose was well controlled. 1 case of vegetative state (severe new infarction during treatment, still in hospital), 2 cases died (heart failure aggravated, stay in bed for a long time, a variety of diabetic complications). Conclusion the disability rate of large area cerebral infarction itself is high, and the mortality and disability rate of diabetes mellitus complicated with heart failure are higher. In the course of treatment, we should mainly control brain edema, pay attention to the management of heart failure and the adjustment of blood glucose, cooperate with suitable limb functional exercise and reasonable diet. Minimize the occurrence of complications, improve the quality of life of patients, reduce the burden of family and society.
