【摘要】：目的探究橋小腦角腦膜瘤導(dǎo)致繼發(fā)性三叉神經(jīng)痛的手術(shù)入路,評估乙狀竇后入路的術(shù)后療效。方法回顧性分析21例因三叉神經(jīng)痛治療且發(fā)現(xiàn)有橋小腦角腦膜瘤的患者的臨床資料,16例采用乙狀竇后入路,5例顳下入路,同期進行微血管減壓術(shù)16例。結(jié)果 17例腫瘤完全切除,3例次全切除,1例大部分切除,術(shù)后面部疼痛消失17例,疼痛明顯減輕4例。術(shù)后隨訪3~12個月,無顏面部疼痛或腫瘤復(fù)發(fā)病例。結(jié)論乙狀竇后入路可有效切除絕大部分橋小腦角腦膜瘤,緩解疼痛,減少術(shù)后并發(fā)癥,是橋小腦角區(qū)重要的手術(shù)入路。
[Abstract]:Objective to investigate the surgical approach to secondary trigeminal neuralgia caused by cerebellopontine angle meningioma and to evaluate the postoperative effect of retrosigmoid approach. Methods the clinical data of 21 patients with cerebellopontine angle meningioma treated with trigeminal neuralgia were retrospectively analyzed. 16 cases were treated by retrosigmoid sinus approach and 5 cases by infratemporal approach. 16 cases underwent microvascular decompression at the same time. Results the tumor was completely resected in 17 cases, subtotal resection in 3 cases, partial resection in 1 case, facial pain disappeared in 17 cases, and the pain was obviously alleviated in 4 cases. No cases of facial pain or tumor recurrence were followed up for 3 months or 12 months. Conclusion retrosigmoid approach can effectively remove most of cerebellopontine angle meningiomas, relieve pain and reduce postoperative complications. It is an important surgical approach in cerebellopontine angle region.
