腦梗死患者同型半胱氨酸與血脂相關(guān)性及其診斷價(jià)值

【摘要】:目的研究同型半胱氨酸(Hcy)與血脂的相關(guān)性及Hcy在腦梗死中的診斷價(jià)值。方法將我院神經(jīng)內(nèi)科住院患者218例,根據(jù)頭顱CT或MRI檢查,分為腦梗組109例,非腦梗組109例,并以健康體檢者60例作對(duì)照組,各組均行Hcy和血脂TC、TG、HDL-c、LDL-c檢測(cè)。以Hcy作診斷試驗(yàn)指標(biāo),頭顱CT和MRI檢查作為診斷的金標(biāo)準(zhǔn),測(cè)量其敏感性、特異性、預(yù)測(cè)值和似然比。結(jié)果兩組TC和Hcy均高于對(duì)照組(P0.01),腦梗組HDL-c低于對(duì)照組(P0.05),且Hcy與HDL-c呈負(fù)相關(guān)(r=-0.144,P0.05),表明高Hcy血癥與HDL的降低有關(guān)。Hcy試驗(yàn)敏感性為42.20%、特異性為54.13%、陽(yáng)性預(yù)測(cè)值和陰性預(yù)測(cè)值分別為47.92%和48.36%、陽(yáng)性似然比和陰性似然比比值分別為0.97和1.07。結(jié)論 Hcy與血脂有一定的相關(guān)性,但Hcy對(duì)腦梗死的診斷價(jià)值較低,不能作為腦梗死診斷的生物學(xué)標(biāo)記物。
[Abstract]:Objective to study the relationship between homocysteine (Hcy) and serum lipids and the diagnostic value of Hcy in cerebral infarction. Methods according to the examination of CT or MRI, 218 inpatients were divided into cerebral infarction group (n = 109) and non-cerebral infarction group (n = 109), and 60 healthy subjects were used as control group. Hcy and blood lipid TC,TG,HDL-c,LDL-c were measured in all groups. The sensitivity, specificity, predictive value and likelihood ratio were measured by using Hcy as diagnostic index and CT and MRI as gold standard for diagnosis. Results the levels of TC and Hcy in the two groups were higher than those in the control group (P0.01), the HDL-c in the cerebral infarction group was lower than that in the control group (P0.05), and there was a negative correlation between Hcy and HDL-c (r = 0.144, P0.05). The sensitivity and specificity of homocysteine test were 42.20%, 54.13%, 47.92% and 48.36% respectively, the positive predictive value and negative predictive value were 47.92% and 48.36%, respectively, and the positive predictive value and negative predictive value were 47.92% and 48.36%, respectively. The positive likelihood ratio and negative likelihood ratio were 0.97 and 1.07, respectively. Conclusion there is a certain correlation between Hcy and blood lipid, but Hcy is of low value in the diagnosis of cerebral infarction and can not be used as a biological marker for the diagnosis of cerebral infarction.
