運(yùn)用三維步態(tài)分析評(píng)價(jià)下肢機(jī)器人對(duì)偏癱患者步行能力的影響

【摘要】:目的 偏癱步態(tài)是腦卒中患者的最大后遺癥之一,嚴(yán)重影響了患者的康復(fù)治療效果和生活自理能力。步行能力的康復(fù)是臨床上研究的重點(diǎn)和熱點(diǎn),現(xiàn)在臨床應(yīng)用的下肢機(jī)器人訓(xùn)練相比傳統(tǒng)步行訓(xùn)練是否效果更佳,兩種方法在效果上優(yōu)劣能否量化評(píng)估,本研究創(chuàng)新性的使用三維步態(tài)分析這一先進(jìn)、客觀的評(píng)估工具,從多個(gè)不同步態(tài)參數(shù)多角度探討下肢機(jī)器人對(duì)偏癱患者步行能力的影響方法將60例符合要求的偏癱病人分為實(shí)驗(yàn)組和對(duì)照組,兩組各30人,60人都進(jìn)行傳統(tǒng)康復(fù)的藥物治療和物理治療,此外實(shí)驗(yàn)組采用下肢機(jī)器人進(jìn)行步行訓(xùn)練,對(duì)照組按照傳統(tǒng)步行訓(xùn)練方法。兩種訓(xùn)練時(shí)長都為每次30分鐘,每周訓(xùn)練5天,4周為1個(gè)療程,共訓(xùn)練2個(gè)療程、療程問歇3天。在治療前和訓(xùn)練8周后,使用三維步態(tài)分析評(píng)估分析兩組的時(shí)間參數(shù)、時(shí)相參數(shù)、關(guān)節(jié)角度、地反力大小。結(jié)果偏癱患者在使用下肢機(jī)器人訓(xùn)練之后,步速、步頻、跨步長顯著提高,步寬顯著縮;患側(cè)下肢擺動(dòng)相百分比增加,患側(cè)支撐相百分比、健側(cè)與患側(cè)支撐相比值、雙支撐相百分比減;髖關(guān)節(jié)和膝關(guān)節(jié)的最大活動(dòng)角度增大,垂直和向前兩項(xiàng)地反力峰值增大。而且與對(duì)照組相比,變化有顯著性(P0.05)結(jié)論對(duì)偏癱患者來說,相比傳統(tǒng)步行訓(xùn)練,下肢機(jī)器人訓(xùn)練可以更有效地改善偏癱患者的步行能力。
[Abstract]:Objective Hemiplegic gait is one of the biggest sequelae of stroke patients, which seriously affects the rehabilitation effect and self-care ability of patients. The rehabilitation of walking ability is the focus and focus of clinical research. Whether the clinical training of lower limb robot is more effective than traditional walking training, can the two methods be evaluated quantitatively. The innovative use of three-dimensional gait analysis as an advanced, objective evaluation tool, The influence of lower limb robot on walking ability of patients with hemiplegia from different gait parameters 60 patients with hemiplegia were divided into two groups: experimental group and control group. 60 patients were treated with traditional rehabilitation medicine and physical therapy. In addition, the experimental group was trained by lower limb robot, while the control group was trained by traditional walking training method. The duration of each training was 30 minutes, 5 days a week, 4 weeks as a course of treatment, a total of 2 courses of training, the course of three days of rest. Three dimensional gait analysis was used to evaluate and analyze the time parameters, phase parameters, joint angle and ground reaction force of the two groups before treatment and 8 weeks after training. Results after using lower limb robot training, the speed, frequency and stride length of the hemiplegic patients were significantly increased, and the width of the step was significantly reduced. The percentage of swinging phase of the affected lower extremity increased, the percentage of the support phase of the affected side increased, the percentage of the double bracing phase of the healthy side decreased compared with the value of the affected side support, and the maximum motion angle of the hip joint and the knee joint increased, and the peak value of the vertical and forward ground reaction increased. And compared with the control group the change was significant (P0.05) conclusion compared with the traditional walking training lower limb robot training can improve the walking ability of hemiplegic patients more effectively.
