顳葉癲癇患者海馬CA3和內(nèi)嗅皮層網(wǎng)絡(luò)的癲癇尖波對(duì)theta節(jié)律抑制作用的量化評(píng)估
[Abstract]:Hippocampal CA3 and entorhinal cortex (entorhinal cortex,EC) network are one of the key circuits in the study of hippocampal cognitive function and temporal lobe epilepsy. The study of the inhibitory effect of sharp waves on the rhythm of hippocampal network theta (48 Hz) is helpful to reveal the mechanism of the effect of epilepsy on cognitive function. In the past, at the network level, the inhibitory effect was often evaluated quantitatively by brain slices. The purpose of this paper is to establish a method to evaluate the inhibitory effect of epileptic cusp on theta rhythm depending on the depth of brain field potential. In 4 patients with temporal lobe epilepsy who were in the condition of rapid eye movement (rapid eyes movement,REM) sleep before operation, the interictal sporadic sharp wave (sporadic spikes,SSs) EEG and the non-sharp wave transient EEG between the two adjacent SSs were selected from the cortical electrode records. The sharp waves appear only in EC, at CA3, or in CA3 and EC at the same time. Using Gabor wavelet and Hilbert transform to calculate the theta energy before and after the sharp wave and the transient phase without the sharp wave, and to calculate the breaking degree of the theta rhythm in the non-sharp wave transient phase. The results showed that: (1) the energy of theta could be instantly decreased by the sharp wave, and the decrease of CA3 and EC was the most severe, and the inhibitory effect was the strongest at the time of synchronous sharp wave of CA3 and EC; (2) the theta energy decreases and the theta rhythm disappears during the non-sharp wave transient period, which results in the rhythm breaking, indicating that the inhibition action is continuing, and the degree of the fracture is consistent with that near the sharp wave. (3) the decrease of theta energy level in 3 patients without sharp wave transient phase was consistent with the inhibitory effect near the sharp wave, but not in 1 case. The results suggest that SSs can produce transient and direct inhibitory effect on theta rhythm, which can be sustained during the non-sharp wave transient period, and can be reflected by the breaking degree of theta rhythm. For the first time, the local field potential has proved that the inhibitory effect of epileptic sharp waves on theta rhythm can be evaluated by the breaking degree of EEG rhythm without sharp waves, which provides a quantitative analysis method for the use of EEG to measure the inhibitory effect of epileptic sharp waves.
