【摘要】：目的研究伴與不伴快動眼睡眠行為障礙(rapid eye movement sleep behavior disorder,RBD)的帕金森病(Parkinson disease,PD)患者的震顫特征及多巴反應性。方法根據2014年國際睡眠障礙分類第三版RBD的臨床最低診斷標準,本研究采用RBD篩查問卷(RBD screening questionnaire,RBDSQ)量表來診斷臨床很可能RBD(clinically probable RBD,cpRBD),將PD患者分為伴有cpRBD的PD(PD+cpRBD)與不伴有cpRBD的PD(PD-cpRBD)兩組。對入組患者進行一般資料的收集,采用修訂的H-Y分級、統(tǒng)一帕金森評分量表3.0版運動檢查部分(UPDRS-Ⅲ)、MDS-UPDRS震顫量表對患者的運動功能進行評估,并且分別對兩組患者首發(fā)側肢體姿勢性震顫、動作性震顫及靜止性震顫的幅度進行評分,比較兩組患者一般資料及震顫特征的差異性。對所有患者行急性左旋多巴沖擊試驗,將兩組患者UPDRS-Ⅲ及MDS-UPDRS震顫量表評分最大改善率進行比較。結果共納入42例伴有震顫的PD患者,PD+cpRBD組19例,PD-cpRBD組共23例,兩組患者在性別、年齡、發(fā)病年齡、病程、關期UPDRS-Ⅲ評分及H-Y分級方面均無明顯差異(P0.05)。與PD-cpRBD組相比,PD+cpRBD組關期震顫評分明顯增高(t=2.379,P=0.022),震顫癥狀由首發(fā)側肢體進展至對側肢體的時間短(u=-2.133,P=0.033),首發(fā)側肢體靜止性震顫幅度大(u=-2.956,P=0.003),動作性震顫幅度大(u=-2.657,P=0.008)�？诜笮喟�/芐絲肼(200/50 mg)后,PD-cpRBD組的UPDRS-Ⅲ及震顫評分最大改善率均明顯高于PD+cpRBD組(UPDRS-Ⅲ最大改善率u=-3.134,P=0.002;震顫評分最大改善率t=-3.189,P=0.003)。結論本研究表明,伴有cpRBD的PD患者震顫程度相對較重,以靜止性震顫和動作性震顫為主,由首發(fā)側肢體進展至對側肢體的時間相對較短,對左旋多巴的反應性較差。
