【摘要】：目的探討低血鈣濃度與腦出血患者血腫體積的關系。方法前瞻性連續(xù)性納入2012年1月至2014年10月發(fā)病24 h內入住四川大學華西醫(yī)院神經外科腦出血患者870例,完成血清鈣濃度實驗室檢查和頭部CT檢查,并收集基線資料及實驗室檢查結果。根據實驗室血鈣濃度正常參考值,將患者分為低血鈣組(2.1 mmol/L)193例,及正常血鈣組(2.1~2.7 mmol/L)677例。采用Spearman相關性分析,分析血鈣濃度與患者入院時血腫體積的相關性。結果 (1)低血鈣組與正常血鈣組比較,男性患者比例高[73.6%(142例)比66.0%(447例)],格拉斯哥昏迷評分中位數分值更低(9分比11分),血腫體積中位數更大(33.86 cm3比21.69 cm3),差異均有統計學意義(均P0.05)。(2)Spearman相關性分析顯示,腦出血患者入院時低血鈣水平與血腫體積呈弱負相關(r=-0.113,P0.01)。結論該研究提示腦出血患者入院時低血鈣濃度男性居多、病情重及血腫體積大,并且低血鈣濃度與血腫體積呈潛在負相關性。
[Abstract]:Objective To investigate the relationship between hypocalcemia and hematoma volume in patients with intracerebral hemorrhage.Methods A prospective cohort of 870 patients with intracerebral hemorrhage admitted to the Department of Neurosurgery of West China Hospital of Sichuan University from January 2012 to October 2014 was conducted. According to the normal reference value of serum calcium concentration in laboratory, 193 patients were divided into hypocalcemia group (2.1 mmol/L) and 677 normal serum calcium group (2.1-2.7 mmol/L). Spearman correlation analysis was used to analyze the correlation between serum calcium concentration and hematoma volume at admission. The median score of Glasgow coma score was lower (9 points to 11 points) and the median hematoma volume was larger (33.86 cm 3 to 21.69 cm 3) than 66.0% (447 cases). The difference was statistically significant (all P 0.05). (2) Spearman correlation analysis showed that there was a weak negative correlation between hypocalcemia and hematoma volume at admission in patients with cerebral hemorrhage (r = - 0.113, P 0.01). This study suggests that the majority of patients with cerebral hemorrhage were male with low serum calcium concentration at admission. The condition was severe and the hematoma volume was large.
