腔隙性腦梗死患者經(jīng)顱多普勒血流參數(shù)與神經(jīng)損傷及炎癥反應(yīng)程度、Hcy代謝的相關(guān)性

【摘要】:目的:研究腔隙性腦梗死患者經(jīng)顱多普勒血流參數(shù)與神經(jīng)損傷及炎癥反應(yīng)程度、Hcy代謝的相關(guān)性。方法:選擇在我院確診為腔隙性腦梗死(LI)的88例患者作為LI組,另取同期體檢的100例健康志愿者作為對照組。采用經(jīng)顱多普勒超聲測定大腦中動脈以及基底動脈的血流參數(shù),采集血清并測定神經(jīng)損傷分子、炎癥反應(yīng)指標(biāo)、Hcy代謝指標(biāo)。結(jié)果:LI組大腦中動脈的MFV、PI以及基底動脈的MFV、PI均顯著高于對照組;LI組血清中NSE、S100B、UCH-L1、ox-LDL、MMP-9、IL-18、sCD40L、Hcy的含量顯著高于對照組且與大腦中動脈、基底動脈的MFV、PI呈正相關(guān),葉酸、維生素B12的含量顯著低于對照組且與大腦中動脈、基底動脈的MFV、PI呈負相關(guān)。結(jié)論:腔隙性腦梗死患者大腦中動脈以及基底動脈的血流加快、搏動增強,顱內(nèi)動脈血流參數(shù)的改變與神經(jīng)損傷程度、炎癥反應(yīng)程度、Hcy代謝密切相關(guān)。
[Abstract]:Objective: to study the correlation between transcranial Doppler blood flow parameters and Hcy metabolism in patients with lacunar cerebral infarction. Methods: 88 patients with lacunar cerebral infarction (LI) were selected as Li group and 100 healthy volunteers as control group. The blood flow parameters of the middle cerebral artery and the basilar artery were measured by transcranial Doppler ultrasound. Results in the middle cerebral artery (MCA) and basilar artery (BAA) of the control group, the MFV Pi of the middle cerebral artery and the basilar artery were significantly higher than those of the control group, and the contents of MMP-9IL-18 and CD40LHcy in the serum of the control group were significantly higher than those in the control group, and were positively correlated with the MFVPI of the middle cerebral artery and the basilar artery, folic acid, and folic acid. The content of vitamin B12 was significantly lower than that of the control group and negatively correlated with the MFV Pi of the middle cerebral artery and basilar artery. Conclusion: in patients with lacunar cerebral infarction, the blood flow of middle cerebral artery and basilar artery is accelerated, pulsatility is enhanced, and the change of blood flow parameters of intracranial artery is closely related to the degree of nerve injury and the degree of inflammatory reaction and Hcy metabolism.
