【摘要】：目的探討血清中性粒細胞與淋巴細胞比值(NLR)對高血壓病性腦出血患者發(fā)生卒中相關(guān)性肺炎(SAP)的預(yù)測價值。方法采用回顧性研究方法,選取2015年1月-2016年11月南華大學附屬南華醫(yī)院重癥醫(yī)學科收治的年齡在50~80歲,且住院時間≥7 d的118例高血壓病性腦出血患者。根據(jù)患者入院后7 d是否發(fā)生SAP將所有患者分為SAP組及非SAP組。比較兩組患者的一般臨床信息,分析入院時APACHEⅡ評分與入院后第1、3及5天的NLR值及白細胞(WBC)值的相關(guān)性,并評價兩組患者在入院后第1、3及5天的WBC及NLR的變化以及組間差異,繪制受試者工作特征曲線(ROC曲線),評價不同時間各個指標對SAP發(fā)生的預(yù)測價值。結(jié)果入院時APACHEⅡ評分與入院后第3、5天的NLR水平第3天(r=0.360,P=0.000);第5天(r=0.390,P=0.000)及WBC水平第3天(r=0.220,P=0.015);第5天(r=0.200,P=0.031)呈正相關(guān)。SAP患者在入院后的第1、3及5天的NLR值分別為(10.89±3.05)、(12.42±3.75)及(9.28±3.31)。其中,第3天的NLR值達到最高峰。且在第3、5天中,SAP組的WBC與NLR值均大于同期的非SAP組(P0.05)。入院后第3、5天的NLR及WBC均能很好地預(yù)測SAP(P0.05),2個時間的預(yù)測價值差異無統(tǒng)計學意義。且NLR的曲線下面積(AUC)(第3天為0.943,第5天為0.924)大于同期WBC的AUC值(第3天為0.823,第5天為0.809)。將NLR與WBC聯(lián)合后不能增加曲線下面積。結(jié)論入院后第3、5天的NLR值對于評估SAP在臨床上有很好的預(yù)測價值。
