【摘要】：目的分析脂蛋白相關(guān)磷脂酶(Lp-PL)A2在老年急性缺血性腦卒中早期診斷中的臨床價(jià)值。方法急性缺血性腦卒中患者46例為觀察組,同期健康體檢者50例為對(duì)照組。檢測(cè)對(duì)比兩組外周血Lp-PLA2、C-反應(yīng)蛋白(CRP)、白細(xì)胞介素(IL)-10及腫瘤壞死因子(TNF)-α水平,并分析各項(xiàng)指標(biāo)對(duì)急性缺血性腦卒中的早期診斷價(jià)值。結(jié)果兩組外周血IL-10水平無(wú)顯著性差異(P0.05)。觀察組外周血TNF-α、Lp-PLA2及CRP水平均顯著高于對(duì)照組(P0.05)。觀察組外周血指Lp-PLA2與TNF-α(r=0.908,P0.05)及CRP(r=0.819,P0.05)均呈正相關(guān)。受試者工作特征(ROC)曲線分析顯示,以181.50 ng/ml為截點(diǎn)值,Lp-PLA2診斷急性缺血性腦卒中的敏感性為84.78%,特異性為84.00%,曲線下面積為0.82,優(yōu)于TNF-α及CRP。結(jié)論 Lp-PLA2水平在老年急性缺血性腦卒中患者病程早期即可升高,具有一定輔助診斷價(jià)值。
[Abstract]:Objective to analyze the clinical value of lipoprotein associated phospholipase A 2 (Lp-PL A 2) in the early diagnosis of acute ischemic stroke in the elderly. Methods 46 patients with acute ischemic stroke were treated as observation group and 50 healthy persons as control group. The levels of Lp-PLA2C- reactive protein (CRP), (IL)-10 and tumor necrosis factor (TNF)-偽 in peripheral blood of the two groups were detected and compared, and the value of each index in the early diagnosis of acute ischemic stroke was analyzed. Results there was no significant difference in IL-10 level between the two groups (P 0.05). The levels of TNF- 偽 Lp-PLA2 and CRP in the observation group were significantly higher than those in the control group (P0.05). In the observation group, there was a positive correlation between Lp-PLA2 and TNF- 偽 (r = 0.908, P 0.05) and CRP (r = 0.819, P 0.05). The (ROC) curve analysis showed that the sensitivity and specificity of Lp-PLA2 in the diagnosis of acute ischemic stroke with 181.50 ng/ml as the cut-off point were 84.78 and 84.00, and the area under the curve was 0.82which was superior to TNF- 偽 and CRP. Conclusion the level of Lp-PLA2 in aged patients with acute ischemic stroke can be increased at the early stage, and it is valuable in the diagnosis of acute ischemic stroke.
