【摘要】：目的:探討大劑量甲潑尼龍沖擊治療急性脊髓炎的療效及其對(duì)患者神經(jīng)功能恢復(fù)、錐體束傳導(dǎo)功能和感覺(jué)障礙的影響。方法:116例急性脊髓炎患者隨機(jī)分為觀察組和對(duì)照組,每組58例。兩組患者均給予常規(guī)治療,在此基礎(chǔ)上對(duì)照組患者給予地塞米松磷酸鈉注射液0.4 mg/(kg·d),靜脈滴注,每天1次,7 d后改服醋酸潑尼松片60 mg,每天1次并逐漸減量至5 mg/d。觀察組患者給予注射用甲潑尼龍琥珀酸鈉1 000 mg,靜脈滴注,每天1次,7 d后改服醋酸潑尼松片60 mg,每天1次并逐漸減量至5 mg/d。兩組療程均為1個(gè)月。比較兩組患者神經(jīng)功能恢復(fù)時(shí)間、錐體束傳導(dǎo)功能、感覺(jué)障礙評(píng)分、臨床療效和不良反應(yīng)發(fā)生情況。結(jié)果:治療后觀察組患者總有效率顯著高于對(duì)照組(93.10%vs.79.31%),肌力改善≥2級(jí)時(shí)間、下地行走時(shí)間、排尿排便功能恢復(fù)時(shí)間均顯著短于對(duì)照組,差異均有統(tǒng)計(jì)學(xué)意義(P0.05)。治療前,兩組患者上、下肢錐體束傳導(dǎo)潛伏期、波幅和感覺(jué)、痛覺(jué)、觸覺(jué)損傷評(píng)分比較,差異均無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。治療后,兩組患者上、下肢錐體束傳導(dǎo)潛伏期顯著低于同組治療前,且觀察組顯著低于對(duì)照組;兩組患者錐體束傳導(dǎo)波幅和感覺(jué)、痛覺(jué)、觸覺(jué)損傷評(píng)分顯著高于同組治療前,且觀察組顯著高于對(duì)照組,差異均有統(tǒng)計(jì)學(xué)意義(P0.05)。兩組患者不良反應(yīng)發(fā)生率比較,差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:在常規(guī)治療基礎(chǔ)上,大劑量甲潑尼龍沖擊治療急性脊髓炎療效較好,有助于改善患者錐體束傳導(dǎo)功能,緩解感覺(jué)障礙程度,促進(jìn)神經(jīng)功能恢復(fù),安全性亦較好。
[Abstract]:Objective: to investigate the effect of high dose methylprednisolone on the recovery of nerve function, pyramidal bundle conduction function and sensory disturbance in patients with acute myelitis. Methods 116 patients with acute myelitis were randomly divided into observation group and control group with 58 cases in each group. The patients in both groups were given routine treatment. The control group was given dexamethasone sodium phosphate injection 0.4 mg/ (kg d), intravenous drip once a day for 7 days before taking prednisone acetate tablets 60 mg once a day and gradually reduced to 5 mg / d.
