【摘要】：目的探討頸動(dòng)脈粥樣硬化與進(jìn)展性缺血性腦卒中的關(guān)系。方法采用彩色多普勒超聲儀對(duì)564例缺血性腦卒中患者的頸動(dòng)脈進(jìn)行評(píng)估,比較進(jìn)展性缺血性腦卒中和非進(jìn)展性缺血性腦卒中患者的頸動(dòng)脈粥樣硬化特征和程度。結(jié)果564例缺血性腦卒中患者有135例為進(jìn)展性缺血性腦卒中(23.8%);在重度頸動(dòng)脈粥樣硬化110例中,有49例(44.5%)發(fā)展為進(jìn)展性卒中;在重度頸動(dòng)脈狹窄95例中,有48例(50.5%)發(fā)展為進(jìn)展性卒中;在病理表現(xiàn)為潰瘍斑86例中,有47例(54.7%)發(fā)展為進(jìn)展性卒中:無頸動(dòng)脈粥樣硬化或伴輕度頸動(dòng)脈粥樣硬化的缺血性腦卒中患者,僅9%～10%發(fā)生進(jìn)展性卒中。經(jīng)Logistic回歸分析發(fā)現(xiàn),頸動(dòng)脈粥樣硬化程度、狹窄程度以及潰瘍斑與進(jìn)展性缺血性卒中的發(fā)生成正相關(guān)。結(jié)論頸動(dòng)脈粥樣硬化與進(jìn)展性缺血性腦卒中的發(fā)生密切相關(guān),頸動(dòng)脈粥樣硬化的嚴(yán)重程度可作為進(jìn)展性缺血性腦卒中的預(yù)測(cè)指標(biāo)。
[Abstract]:Objective to investigate the relationship between carotid atherosclerosis and progressive ischemic stroke. Methods the carotid artery of 564 patients with ischemic stroke were evaluated by color Doppler ultrasound, and the characteristics and degree of carotid atherosclerosis were compared between progressive ischemic stroke and non-progressive ischemic stroke. Results among 564 patients with ischemic stroke, 135 had progressive ischemic stroke, 49 out of 110 patients with severe carotid atherosclerosis, 49 with progressive stroke and 95 with severe carotid stenosis. In 48 patients with progressive stroke, 47 of 86 patients with ulcer plaque developed progressive stroke: patients without carotid atherosclerosis or ischemic stroke with mild carotid atherosclerosis, Only 9% had progressive stroke. Logistic regression analysis showed that the degree of carotid atherosclerosis, stenosis and ulcer plaque were positively correlated with the occurrence of progressive ischemic stroke. Conclusion carotid atherosclerosis is closely related to the occurrence of progressive ischemic stroke, and the severity of carotid atherosclerosis can be used as a predictor of progressive ischemic stroke.
