血漿miR-27b、miR-10b與腦動(dòng)脈粥樣硬化及脂代謝異常的關(guān)系

【摘要】:目的:探討血漿miR-27b、miR-10b與腦動(dòng)脈粥樣硬化及脂代謝異常的關(guān)系。方法:選取47例同期健康查體者(對(duì)照組)、38例無癥狀腦動(dòng)脈粥樣硬化患者(AS組)、52例大動(dòng)脈粥樣硬化性腦梗死患者(LAA組)和52例小動(dòng)脈閉塞性腦梗死患者(SAO組),實(shí)時(shí)熒光定量PCR(qRT-PCR)檢測(cè)血漿中miR-27b和miR-10b的表達(dá)水平;為評(píng)價(jià)腦動(dòng)脈粥樣硬化病變廣泛程度,將對(duì)照組設(shè)為非動(dòng)脈粥樣硬化組,LAA組及AS組設(shè)為動(dòng)脈粥樣硬化組,將動(dòng)脈粥樣硬化組按照單支、雙支、多支(三支及以上)病變分為3個(gè)亞組,Spearman相關(guān)分析miR-27b和miR-1Ob的表達(dá)水平與腦動(dòng)脈粥樣硬化血管病變廣泛程度的相關(guān)性;應(yīng)用Pearson相關(guān)分析血漿miR-27b和miR-10b的表達(dá)水平與血脂之間的相關(guān)性;應(yīng)用受試者工作特征(ROC)曲線評(píng)估m(xù)iR-27b和miR-10b對(duì)腦動(dòng)脈粥樣硬化識(shí)別的準(zhǔn)確性。結(jié)果:miR-27b、miR-10b和TC水平在動(dòng)脈粥樣硬化組表達(dá)水平顯著高于對(duì)照組及 SAO 組(P0.05),而在對(duì)照組與 SAO 組(P0.05)、AS 組與 LAA 組(P0.05)之間差異均無統(tǒng)計(jì)學(xué)意義,提示miR-27b、miR-10b與腦動(dòng)脈粥樣硬化有關(guān),且梗死對(duì)其影響較小;Spearman相關(guān)分析,結(jié)果顯示miR-27b及miR-10b其血漿表達(dá)水平與腦動(dòng)脈粥樣硬化血管病變廣泛程度有相關(guān)性(r=-0.571,P0.001;r=-0.521,P0.001)。Pearson相關(guān)分析,結(jié)果顯示miR-27b及miR-10b的血漿表達(dá)水平在動(dòng)脈粥樣硬化組及非動(dòng)脈粥樣硬化組中均與TC水平均有相關(guān)性(r動(dòng)27=-0.859,P動(dòng)270.001;r 動(dòng) 10=-0.594,P 動(dòng) 100.001;r 非 27=-0.426,P 非 27=0.003;r 非 10=-0.653,P非100.001)。利用ROC曲線評(píng)估m(xù)iR-27b和miR-10b對(duì)腦動(dòng)脈粥樣硬化的識(shí)別效能,ROC 曲線下面積分別為 0.932(95%CI:0.884-0.980,P0.001);0.924(95%CI:0.870-0.979,P0.001),cut-off 值為 5.48;5.82,靈敏度和特異度分別為 89.4%、91.1%;85.1%、96.7%。結(jié)論:miR-27b及miR-10b的血漿表達(dá)水平在AS組及LAA組中較對(duì)照組及SAO組明顯上調(diào),但在AS組與LAA組、對(duì)照組與SAO組之間無明顯統(tǒng)計(jì)學(xué)差異,而且,miR-27b及miR-10b在血漿中的表達(dá)水平與腦動(dòng)脈粥樣硬化廣泛程度具有明顯相關(guān)。這說明,miR-27b及miR-10b不僅與腦動(dòng)脈粥樣硬化有關(guān),并且當(dāng)miR-27b及miR-10b在血漿中表達(dá)越高時(shí),腦動(dòng)脈粥樣硬化的病變?cè)綇V泛。
[Abstract]:Aim: to investigate the relationship between miR-27b miR-10b and cerebral atherosclerosis and abnormal lipid metabolism. Methods: 47 healthy examiners (38 asymptomatic cerebral atherosclerosis patients in the control group) and 52 patients with arteriosclerotic cerebral infarction (ACI group) and 52 small artery occlusive cerebral infarction patients (SAO group) were selected. The expression levels of miR-27b and miR-10b in plasma were detected by PCR qRT-PCR. In order to evaluate the extent of cerebral atherosclerosis, the control group was divided into non-atherosclerosis group (LAA) and as group (group A) as atherosclerosis group, and atherosclerosis group was divided into two groups according to single and double branches. Spearman correlation Analysis between the expression of miR-27b and miR-1Ob and the extent of atherosclerotic vascular lesions; Pearson correlation was used to analyze the correlation between the expression of miR-27b and miR-10b in plasma and serum lipid, and the accuracy of miR-27b and miR-10b in the recognition of cerebral atherosclerosis was evaluated by ROC- curve. Results the expression levels of miR-27b miR-10b and TC in atherosclerosis group were significantly higher than those in control group and SAO group (P 0.05), but there was no significant difference between control group and SAO group (P 0.05), suggesting that miR-27b miR-10b was associated with cerebral artery atherosclerosis. The results of Spearman correlation analysis showed that the plasma expression levels of miR-27b and miR-10b were correlated with the extent of atherosclerotic vascular lesions. The results showed that the plasma levels of miR-27b and miR-10b were correlated with TC levels in atherosclerosis group and non-atherosclerotic group. The plasma levels of miR-27b and miR-10b were correlated with TC level in atherosclerotic group and non-atherosclerosis group. The plasma levels of miR-27b and miR-10b were correlated with TC level in atherosclerotic group and non-atherosclerosis group. The ROC curve was used to evaluate the recognition effectiveness of miR-27b and miR-10b for atherosclerosis. The area under the ROC curve was 0.932 ~ 95% CI: 0.884-0.980% (P 0.001), and the cut-off value was 5.485.82, and the sensitivity and specificity were 89.44 ~ 91.1% and 96.771, respectively. Conclusion the plasma levels of miR-27b and miR-10b in as group and LAA group were significantly higher than those in control group and SAO group, but there was no significant difference between as group and LAA group, control group and SAO group. The expression levels of miR-27b and miR-10b in plasma were significantly correlated with the extent of cerebral atherosclerosis. These results suggest that miR-27b and miR-10b are not only associated with atherosclerosis, but also that the higher the expression of miR-27b and miR-10b in plasma, the wider the lesion of cerebral atherosclerosis.
