【摘要】：目的探討簡化院內(nèi)救治流程對急性缺血性腦卒中患者靜脈溶栓效果的影響。方法選取2014年1月~2015年12月因急性缺血性腦卒中行重組組織型纖溶酶原激活劑(rt-PA)靜脈溶栓治療的72例患者,其中2014年收治的35例患者為對照組,2015年收治的37例患者為觀察組。對照組采用傳統(tǒng)的院內(nèi)救治流程,觀察組患者采用簡化的院內(nèi)救治流程。比較兩組溶栓7 d時NIHSS評分、改良Rankin評分、癥狀性顱內(nèi)出血發(fā)生率、病死率。結(jié)果觀察組DNT、發(fā)病至溶栓時間、就診至CT判讀時間、就診至生化報告時間、就診至心電圖時間均短于對照組(P均0.01)。與對照組相比,觀察組溶栓7 d時NIHSS評分、改良Rankin評分、癥狀性顱內(nèi)出血發(fā)生率及病死率顯著降低,P均0.05。結(jié)論簡化院內(nèi)救治流程可提高急性缺血性腦卒中溶栓治療的效果。
[Abstract]:Objective to investigate the effect of simplified in-hospital treatment on intravenous thrombolysis in patients with acute ischemic stroke. Methods from January 2014 to December 2015, 72 patients with acute ischemic stroke were treated with recombinant tissue plasminogen activator (rt-PA) intravenous thrombolytic therapy, including 35 patients in 2014 as control group and 37 patients in 2015 as observation group. The patients in the control group were treated with the traditional in-hospital treatment procedure, while the patients in the observation group were treated with the simplified in-hospital treatment procedure. NIHSS score, modified Rankin score, incidence of symptomatic intracranial hemorrhage and mortality were compared between the two groups at 7 days after thrombolysis. Results in the observation group, the time from onset to thrombolysis, from diagnosis to CT interpretation, from visit to biochemical report, and from ECG to the control group were all shorter than those of control group (P < 0.01). Compared with the control group, the NIHSS score, the modified Rankin score, the incidence of symptomatic intracranial hemorrhage and the mortality were significantly decreased in the observation group at 7 days after thrombolysis (P < 0.05). Conclusion simplifying hospital treatment procedure can improve the effect of thrombolytic therapy for acute ischemic stroke.
