【摘要】：目的:探討后循環(huán)腦梗死預(yù)后的相關(guān)影響因素。方法連續(xù)前瞻性登記112例經(jīng)臨床和影像學(xué)證實(shí)的后循環(huán)腦梗死患者的臨床資料與相關(guān)危險(xiǎn)因素并隨訪6個(gè)月,根據(jù)Rankin量表(modified Rankin Scale,mRS)評(píng)分將患者分為預(yù)后良好組(mRS評(píng)分≤2分)和預(yù)后不良組(mRS評(píng)分≥3分),運(yùn)用Logistic回歸分析這些因素與預(yù)后的相關(guān)關(guān)系。結(jié)果112例后循環(huán)腦梗死患者中,預(yù)后良好者82例,嚴(yán)重致殘者27例,死亡3例。多因素分析結(jié)果顯示:2組比較后,預(yù)后不良組,入院時(shí)NIHSS評(píng)分高、高同型半胱氨酸血癥、高脂血癥、呼吸系統(tǒng)感染、混合組病變、血管病變嚴(yán)重者提示預(yù)后不良。結(jié)論入院時(shí)NIHSS評(píng)分高、高同型半胱氨酸血癥、高脂血癥、梗死伴發(fā)呼吸系統(tǒng)感染及多組病變者、血管病變嚴(yán)重者與預(yù)后相關(guān)。
