【摘要】：目的了解腦炎患者中感染人腸道病毒71型病毒(human enterovirus 71,EV71)流行特點,為臨床感染防控提供指導。方法收集812例腦炎患者臨床資料及腦脊液樣本,采用RT-PCR擴增檢測EV71病毒,分析EV71病毒流行情況。結果經RT-PCR檢測,812例腦炎患者中有224例為EV71病毒感染,檢出率27.59%。春、夏、秋、冬四季腦炎患者EV71病毒感染率為33.11%、23.02%、22.22%和26.70%,且春季感染率與其他季節(jié)相比,差異有統(tǒng)計學意義(χ2=7.4741,P=0.0063)。男性和女性患者感染率分別為30.80%和23.08%,感染率差異有統(tǒng)計學意義(χ2=5.8938,P=0.0152)。兒童、青少年、中老年患者感染率分別為26.64%、20.92%和34.57%;中老年患者感染率與其他年齡患者相比,差異有統(tǒng)計學意義(χ2=9.8285,P=0.0017)。224例患者主要臨床表現為突然發(fā)病,發(fā)熱、嘔吐、頭痛、腹瀉、惡心患者分別有182例(81.25%)、164例(73.21%)、155例(69.20%)、63例(28.13%)和26例(11.61%),其他臨床表現不明顯。224例患者經治療后,有144例治愈或好轉出院,出院時均未發(fā)現陽性癥狀或體征。結論春季為EV71病毒腦炎高發(fā)季節(jié),男性中老年患者居多�；颊咧饕憩F為突然發(fā)病,發(fā)熱等癥狀常見。
[Abstract]:Objective to investigate the epidemic characteristics of human enterovirus 71 (enterovirus 71 EV71) in patients with encephalitis and to provide guidance for clinical infection prevention and control. Methods the clinical data of 812 patients with encephalitis and cerebrospinal fluid samples were collected, EV71 virus was detected by RT-PCR amplification, and the prevalence of EV71 virus was analyzed. Results among 812 encephalitis patients detected by RT-PCR, 224 cases were infected with EV71 virus, the detection rate was 27.59%. The infection rate of EV71 virus in the patients with encephalitis in spring, summer, autumn and winter was 33.112.02% and 26.70%, and the infection rate in spring was significantly higher than that in other seasons (蠂 2 7.4741). The infection rates of male and female patients were 30.80% and 23.08, respectively, and the difference was statistically significant (蠂 ~ 2 = 5.8938). The infection rates of children, adolescents and middle-aged and elderly patients were 26.64% and 34.57%, respectively. There was a significant difference in the infection rate between middle and old age patients (蠂 ~ 2 / 9.8285P ~ (0.001717.224). The main clinical manifestations of these patients were sudden onset, fever, vomiting, headache and diarrhea. There were 182 patients with nausea, 164 with 73.21, 155 with 69.20 and 63 with 28. 13. 13) and 26 with 11. 61. The other 224 patients were cured or improved after treatment. No positive symptoms or signs were found at discharge. Conclusion EV71 encephalitis is a high incidence season in spring, male middle-aged and elderly patients are the majority. The main manifestations of the patients were sudden onset, fever and other common symptoms.
