多系統(tǒng)萎縮患者靜息態(tài)下腦活動的局部一致性

【摘要】:目的:采用靜息態(tài)功能磁共振(resting state functional magnetic resonance image,rs-f MRI)方法探討不同亞型多系統(tǒng)萎縮(multiple system atrophy,MSA)患者腦局部一致性(regional homogeneity,ReHo)的變化及其與臨床癥狀嚴重程度的相關(guān)性。方法:對48例MSA患者(MSA-C型28例及MSA-P型20例)和27例健康對照進行rs-f MRI掃描,比較3組間ReHo的差異,采用簡易精神狀態(tài)量表(MMSE)、蒙特利爾認知評估量表(MoCA)、統(tǒng)一的MSA評分量表(UMSARS)等評估臨床癥狀,并分析MSA患者ReHo值與臨床癥狀嚴重程度之間的關(guān)系。結(jié)果:(1)相對于對照組,MSA-C組雙側(cè)楔前葉、左側(cè)海馬、左側(cè)枕中回、小腦蚓部Ⅴ~ⅥReHo增高,左側(cè)小腦前/后葉ReHo減低;MSA-P組雙側(cè)丘腦、左側(cè)尾狀核、腦干、小腦蚓部ⅤReHo增高,左側(cè)小腦前/后葉ReHo減低;相對于MSA-P組,MSA-C組左側(cè)楔前葉、左側(cè)海馬、左側(cè)顳中回ReHo增高,右側(cè)枕中回ReHo減低。(2)MSA-C組雙側(cè)楔前葉與MoCA評分呈負相關(guān),MSA-P組雙側(cè)丘腦與UMSARS-Ⅱ評分呈正相關(guān)。結(jié)論:靜息態(tài)下MSA患者存在局部腦區(qū)的神經(jīng)元活動一致性異常,這些腦區(qū)異?赡芘cMSA的病理機制有關(guān)。
[Abstract]:Objective: to investigate the changes of regional homogeneity in brain of patients with different subtypes of multiple system atrophy (system atrophy) by resting state functional magnetic resonance imageres-f MRIs and its correlation with the severity of clinical symptoms. Methods: rs-f MRI scans were performed in 48 MSA patients with MSA-C type and 20 patients with MSA-P type) and 27 healthy controls. The difference of ReHo among the three groups was compared.
