【摘要】：目的通過(guò)分析類風(fēng)濕關(guān)節(jié)炎(rheumatoid arthritis,RA)合并周?chē)窠?jīng)病變患者的臨床表現(xiàn)、輔助檢查及治療情況,探討RA合并周?chē)窠?jīng)病變的臨床特點(diǎn)。方法將2012年1月至2017年1月蚌埠醫(yī)學(xué)院第一附屬醫(yī)院風(fēng)濕免疫科診斷為RA合并周?chē)窠?jīng)病變的51例住院患者,作為周?chē)窠?jīng)病變組,同時(shí)選取同期住院的年齡、性別匹配的無(wú)周?chē)窠?jīng)病變的51例RA患者相對(duì)照,作為無(wú)周?chē)窠?jīng)病變組。通過(guò)病例對(duì)照研究比較分析兩組之間的臨床表現(xiàn)、輔助檢查及治療情況的差異。應(yīng)用SPSS21.0軟件進(jìn)行統(tǒng)計(jì)分析。結(jié)果(1)RA合并周?chē)窠?jīng)病變發(fā)病率為3.0%(51/1550),周?chē)窠?jīng)病變組的腫脹關(guān)節(jié)計(jì)數(shù)[(11.71±1.93)vs(6.55±1.6),P=0.048];DAS28[(6.28±0.23)vs(5.47±0.29),P=0.036];RF[(538.08±138.48)vs.(230.71±59.61)U/ml,P=0.048];抗CCP抗體[(248.00±52.45)vs(112.98±20.20)IU/ml,P=0.021]均高于無(wú)周?chē)窠?jīng)病變組;周?chē)窠?jīng)病變組補(bǔ)體C3[(0.87±0.04)vs(1.01±0.05)g/L,P=0.043]低于無(wú)周?chē)窠?jīng)病變組,差異具有統(tǒng)計(jì)學(xué)意義(P0.05);而病程、不規(guī)則治療時(shí)間、疼痛關(guān)節(jié)計(jì)數(shù)、關(guān)節(jié)畸形計(jì)數(shù)及時(shí)間、ESR、CRP的差異無(wú)統(tǒng)計(jì)學(xué)意義。(3)周?chē)窠?jīng)病變組在出現(xiàn)神經(jīng)病變前使用的激素日劑量、時(shí)間及使用DMARDs時(shí)間、與無(wú)周?chē)窠?jīng)病變組相比較,差異無(wú)統(tǒng)計(jì)學(xué)意義。結(jié)論RA合并周?chē)窠?jīng)病變?cè)赗A患者中并不罕見(jiàn),腫脹關(guān)節(jié)計(jì)數(shù)多、病情活動(dòng)度高、高滴度的RF、抗CCP抗體及補(bǔ)體C3的下降是RA合并周?chē)窠?jīng)病變的臨床特點(diǎn)。
[Abstract]:Objective to investigate the clinical features of rheumatoid arthritis (RA) with peripheral neuropathy by analyzing the clinical manifestation, adjuvant examination and treatment of rheumatoid arthritis (RA) with peripheral neuropathy. Methods from January 2012 to January 2017, 51 patients with RA complicated with peripheral neuropathy were diagnosed by Rheumatological Immunology Department of the first affiliated Hospital of Bengbu Medical College as peripheral neuropathy group. Sex matched control group of 51 RA patients without peripheral neuropathy. The differences of clinical manifestation, adjuvant examination and treatment between the two groups were analyzed by case-control study.
