肉毒毒素聯(lián)合活血通絡(luò)法治療老年腦卒中后肌肉痙攣的臨床療效研究

【摘要】:目的 :探討肉毒毒素聯(lián)合活血通絡(luò)法治療老年腦卒中后肌肉痙攣的臨床療效。方法:研究對象為醫(yī)院康復(fù)科治療的腦卒中后肌肉痙攣老年患者84例,按隨機數(shù)字表法分組,各42例,對照組予以常規(guī)治療,研究組在常規(guī)治療基礎(chǔ)上予以肉毒毒素聯(lián)合活血通絡(luò)法治療,共治療4周為1療程,采用相應(yīng)標(biāo)準(zhǔn)評價上肢關(guān)節(jié)活動度、肌張力及活動功能狀況,同時對比臨床療效及不良反應(yīng)。結(jié)果:在臨床療效上,對照組有效率71.43%與研究組有效率90.48%數(shù)據(jù)對比,有差異(P0.05);相對于治療前,兩組治療后關(guān)節(jié)活動度升高,肌張力降低,FMA評分和FIM上肢評分均升高,正中神經(jīng)和尺神經(jīng)H反射潛伏期升高,Hmax/Mmax降低,有差異(P0.05);相對于對照組,研究組治療后關(guān)節(jié)活動度較高,肌張力較低,FMA評分和FIM上肢評分較高,正中神經(jīng)和尺神經(jīng)H反射潛伏期較高,Hmax/Mmax較低,有差異(P0.05)。結(jié)論:肉毒毒素聯(lián)合活血通絡(luò)法治療老年腦卒中后肌肉痙攣療效確切,降低上肢肌張力,提高神經(jīng)功能和關(guān)節(jié)活動度。
[Abstract]:Objective: to investigate the clinical effect of botulinum toxin combined with activating blood circulation and dredging collaterals in the treatment of muscle spasm after stroke in the elderly. Methods: 84 elderly patients with muscle spasm after stroke were divided into two groups according to random digital table method. The control group was treated with routine therapy. On the basis of routine treatment, the study group was treated with botulinum toxin combined with activating blood circulation and dredging collaterals for a course of 4 weeks. The corresponding criteria were used to evaluate the upper limb joint motion, muscle tension and activity function. At the same time, the clinical efficacy and adverse reactions were compared. Results: the effective rate of the control group was 71.43% compared with that of the study group (90.48%), there was a difference between the two groups (P 0.05), the joint motion increased, the muscle tension decreased and the FIM upper limb score increased after treatment. The latency of H reflex of median nerve and ulnar nerve was increased and the Hmax / Mmax was decreased (P < 0.05). Compared with the control group, the patients in the study group had higher joint motion, lower muscle tension and higher FIM upper limb score than those in the control group. The latency of H reflex of median nerve and ulnar nerve was higher and Hmax / Mmax was lower than that of median nerve and ulnar nerve (P 0.05). Conclusion: Botulinum toxin combined with activating blood circulation and dredging collaterals is effective in the treatment of muscle spasm after stroke in elderly patients, which can reduce muscle tension of upper limb and improve nerve function and joint activity.
