【摘要】：研究背景及目的:利用手寫(xiě)筆采集手寫(xiě)時(shí)的運(yùn)動(dòng)參數(shù),探討各手寫(xiě)參數(shù)在早中期帕金森病(Parkinson's disease, PD)診斷的作用及其對(duì)前驅(qū)期PD甄別、及運(yùn)動(dòng)癥狀嚴(yán)重程度的量化價(jià)值。方法共有符合英國(guó)腦庫(kù)標(biāo)準(zhǔn)的來(lái)自解放軍總醫(yī)院帕金森專(zhuān)病門(mén)診的PD患者45例(前驅(qū)期患者10例、HY1-2.5級(jí)的早期患者28例、HY3級(jí)的中期患者7例),同教育水平、性別、年齡匹配的健康對(duì)照40例,收集姓名、性別、年齡、受教育程度、共病、PD病程、HY分級(jí),MDS-UPDRS量表Ⅰ、Ⅱ、Ⅲ項(xiàng),PDQ-39,Schwab and England Activities of Daily Living Scale,MMSE, MoC A,BMI, PD 初發(fā)時(shí)亞型,家族史,醫(yī)院焦慮抑郁量表(The Hospital Anxiety and Depression Scale,HADS)臨床資料,并用手寫(xiě)筆在數(shù)位屏上依次完成阿基米德曲線(xiàn)、米字臨摹及固定腕關(guān)節(jié)劃弧線(xiàn)三個(gè)任務(wù),與之相連的計(jì)算機(jī)采集手寫(xiě)的運(yùn)動(dòng)參數(shù)(筆畫(huà)速度、加速度、運(yùn)動(dòng)時(shí)間、震顫幅度、頻率),比較各參數(shù)在健康對(duì)照間和PD組之間的差異,并在PD組內(nèi)將各參數(shù)值與MDS-UPDRSⅢ總分、HY分級(jí)、病程做相關(guān)性分析,最后評(píng)價(jià)各參數(shù)及單一或總體任務(wù)組的診斷效能。結(jié)果①同健康對(duì)照相比,前驅(qū)期PD患者手寫(xiě)的平均運(yùn)動(dòng)速度、加速度峰值、震顫頻率顯著降低于對(duì)照組(2.7±0.3vs3.1±0.7、1.5±0.2vs1.6±0.2、5.1±1.9vs5.7±1.7; p0.05);運(yùn)動(dòng)完成時(shí)間、震顫的振幅均高于對(duì)照組(15.2±2.8vs12.9±3.0、3.2±1.5vs2.5±0.8; p0.05);②同健康對(duì)照相比,PD 患者手寫(xiě)的平均運(yùn)動(dòng)速度、加速度峰值、震顫頻率均顯著降低于對(duì)照組(2.4±0.4vs3.1 ±0.7、1.4±0.2vs1.5±0.3、4.3±1.8vs5.7±1.7; p0.05);運(yùn)動(dòng)完成時(shí)間、震顫的振幅均高于對(duì)照組(16.8±3.2vs12.9±3.0、4.2±1.7vs2.5±0.8; p0.05);③PD組運(yùn)動(dòng)遲緩和震顫各參數(shù)均和MDS-UDPRSⅢ、HY分級(jí)、病程成正相關(guān)(r=0.8131、0.7809、0.7641,p0.05);④基于三項(xiàng)手寫(xiě)任務(wù)的的量化分析整體的診斷模型精確度為90.625%。結(jié)論手寫(xiě)軌跡的量化分析可有效檢測(cè)出前驅(qū)期震顫、運(yùn)動(dòng)遲緩的特征,并對(duì)不同嚴(yán)重程度的帕金森病有客觀(guān)定量?jī)r(jià)值,比傳統(tǒng)MDS-UPDRS量表有快捷、敏感、簡(jiǎn)單、更客觀(guān)的優(yōu)勢(shì),有望成為前驅(qū)期診斷的手段。
[Abstract]:Background and objective: to investigate the role of handwritten parameters in the diagnosis of parkinsonian disease (PDD) in early and middle stage of Parkinson's disease (PDD) by collecting the motion parameters of handwritten pen, and the quantitative value of these parameters in the identification of PD and the severity of motor symptoms in the early and middle stage of Parkinson's disease (PD). Methods A total of 45 PD patients from Parkinson's disease outpatient clinic of PLA General Hospital in accordance with the British brain bank standard (10 cases of prodromal stage patients with HY1-2.5 grade of early stage and 7 cases of intermediate stage of HY3 grade) with the same level of education and sex, were enrolled in this study.
