Rs-EPI彌散加權(quán)成像在顱內(nèi)占位性病變的臨床應(yīng)用研究

【摘要】:目的與常規(guī)單次激發(fā)平面回波(Ss-EPI)彌散加權(quán)成像(DWI)比較,探討讀出方向分段采樣平面回波(Rs-EPI)DWI序列在顱內(nèi)占位性病變的臨床應(yīng)用價(jià)值。方法連續(xù)納入臨床診斷為顱內(nèi)占位性病變的患者21例,采用SEIMENS Skyra 3.0T MRI及20通道的頭頸聯(lián)合線圈,行常規(guī)Ss-EPI DWI和Rs-EPI DWI序列掃描,由2名高年資影像診斷醫(yī)師從對(duì)比噪聲比(CNR)、信噪比(SNR)、偽影、圖像整體質(zhì)量及病變顯示等方面進(jìn)行評(píng)價(jià)分析。兩種序列的圖像質(zhì)量比較采用非參數(shù)Wilcoxon符號(hào)秩和檢驗(yàn),2名閱片者之間一致性檢測(cè)采用Kappa檢驗(yàn)及組內(nèi)相關(guān)系數(shù)(ICC)分析。結(jié)果 21例患者均成功完成兩組序列掃描。Ss-EPI DWI序列的SNR、CNR(130.46±49.10,33.22±18.86)高于Rs-EPI DWI序列(71.58±30.43,17.92±18.72)(P均0.05)。鼻竇旁、乳突氣房旁、額竇旁Rs-EPI DWI畸變偽影(3.67±0.48,3.57±0.50,3.90±0.30)小于Ss-EPI DWI(2.10±0.10,2.33±0.58,2.80±0.60)(P均0.05);Rs-EPI DWI病變顯示優(yōu)于Ss-EPI DWI(3.95±0.22vs.3.19±0.60,P0.05);RsEPI DWI圖像整體質(zhì)量?jī)?yōu)于Ss-EPI DWI(3.86±0.86vs.2.71±0.46,P0.05)。Rs-EPI DWI圖像變形小于SsEPI DWI(0.016±0.021vs.0.037±0.069,P=0.00)。2名閱片者對(duì)兩種序列的主觀及客觀評(píng)價(jià)一致性好[0.74≤Kappa值或ICC≤0.92]。結(jié)論 Rs-EPI DWI序列提高顱內(nèi)占位性病變顯示情況,降低了圖像畸變偽影,可獲得高分辨、高圖像質(zhì)量的彌散加權(quán)圖像。
[Abstract]:Objective to compare with the conventional single excitation plane echo (Ss-EPI) diffusion-weighted imaging (DWI), the clinical value of the read direction piecewise sampling plane echo (Rs-EPI) DWI sequence in the intracranial space occupying lesions was discussed. Methods 21 patients with intracranial space occupying lesions were consecutively diagnosed with SEIMENS Skyra 3.0T MRI and 20 channels of the head and neck. Combined coils, routine Ss-EPI DWI and Rs-EPI DWI sequence scanning, 2 senior image diagnostics were evaluated from contrast to noise ratio (CNR), signal to noise ratio (SNR), artifact, image overall quality and lesion display. The image quality of the two sequences was compared with the non parametric Wilcoxon symbol rank sum test, and the agreement between 2 film readers. Kappa test and intra group correlation coefficient (ICC) analysis were used. Results all 21 patients successfully completed the SNR of the sequence of two sequences, CNR (130.46 + 49.10,33.22 + 18.86) were higher than Rs-EPI DWI sequence (71.58 + 30.43,17.92 + 18.72) (P all 0.05). Paranasal paranasal, papillomastoid gas room, paranasal sinus paranoid aberration artifacts (3.67 + 0.50,3.90 + 0.30) was less than Ss-EPI DWI (2.10 + 0.10,2.33 + 0.58,2.80 + 0.60) (P 0.05), and Rs-EPI DWI lesions showed better than Ss-EPI DWI (3.95 + 0.60, P0.05). The subjective and objective evaluation of the two sequences was consistent with the two sequences, such as good consistency, good [0.74 < Kappa value or ICC < 0.92]. conclusion Rs-EPI DWI sequence to improve the display of intracranial space occupying lesions, reduce the image distortion artifact, and obtain high resolution and high image quality of the diffusion weighted image.
