【摘要】：目的三叉神經(jīng)痛是口腔頜面部常見(jiàn)性及頑固性疾病之一,三叉神經(jīng)痛又分為原發(fā)性三叉神經(jīng)痛和繼發(fā)性三叉神經(jīng)痛。原發(fā)性三叉神經(jīng)痛的發(fā)病原因一直不完全明確,存在多種假說(shuō)及設(shè)想。本研究旨在探討原發(fā)性三叉神經(jīng)痛患者外周血中促炎癥因子白細(xì)胞介素-1β(IL-1β)、白細(xì)胞介素-6(IL-6)和腫瘤壞死因子-α(TNF-α)水平的變化及意義。方法2013年1月~2014年9月就診于安徽省口腔醫(yī)院口腔外科及安徽合肥新民醫(yī)院的原發(fā)性三叉神經(jīng)痛患者30例作為三叉神經(jīng)痛組,所選患者均符合第七版《口腔頜面外科學(xué)》三叉神經(jīng)痛診斷標(biāo)準(zhǔn),其中男13例,女17例;全身某些部位已明確診斷為惡性腫瘤者17例作為惡性腫瘤對(duì)照組,其中男8例,女9例;牙周和根尖周有明顯炎癥者17例作為普通炎癥對(duì)照組,其中男7例,女10例;20例正�；颊咦鳛榻】祵�(duì)照組,其中男9例,女11例;對(duì)照組患者均為就診于安徽省口腔醫(yī)院和安徽省立醫(yī)院的患者。采集各組患者清晨空腹靜脈血5 ml低溫離心(4?C、3 500 r/min、10 min),分離血清后置于-70?C冰箱保存?zhèn)溆?采用雙抗體夾心酶聯(lián)免疫吸附試驗(yàn)(ELISA)檢測(cè)30例三叉神經(jīng)痛患者、17例惡性腫瘤患者、17例普通炎癥患者以及20例健康患者清晨空腹靜脈血血清中促炎癥因子IL-1β、IL-6和TNF-α的表達(dá)水平。按照ELISA試劑盒中顯示的操作步驟進(jìn)行,然后繪制標(biāo)準(zhǔn)曲線,根據(jù)樣品光密度值在標(biāo)準(zhǔn)曲線圖上查出相應(yīng)各促炎癥因子水平,最后進(jìn)行統(tǒng)計(jì)學(xué)處理。結(jié)果三叉神經(jīng)痛患者外周靜脈血血清中促炎癥因子IL-1β、IL-6和TNF-α的表達(dá)水平均高于健康對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(p0.05);而與惡性腫瘤組及普通炎癥組差異無(wú)統(tǒng)計(jì)學(xué)意義。結(jié)論三叉神經(jīng)痛患者外周靜脈血血清中促炎癥因子IL-1β、IL-6和TNF-α的表達(dá)水平明顯升高,提示炎癥因子與三叉神經(jīng)痛發(fā)病有一定關(guān)聯(lián)。
[Abstract]:Objective trigeminal neuralgia is one of the common and obstinate diseases in oral and maxillofacial region. Trigeminal neuralgia is divided into primary trigeminal neuralgia and secondary trigeminal neuralgia. The etiology of primary trigeminal neuralgia is not completely clear, and there are many hypotheses and assumptions. The purpose of this study was to investigate the changes and significance of the levels of interleukin-1 尾 (IL-1 尾), interleukin-6 (IL-6) and tumor necrosis factor- 偽 (TNF- 偽) in peripheral blood of patients with primary trigeminal neuralgia. Methods from January 2013 to September 2014, 30 patients with primary trigeminal neuralgia were treated as trigeminal neuralgia group. All the selected patients met the diagnostic criteria of trigeminal neuralgia in the seventh edition of Oral and Maxillofacial surgery, including 13 males and 17 females. 17 patients with malignant tumors were definitely diagnosed as malignant tumors in some parts of the body, including 8 males. There were 17 cases of periodontal and periapical inflammation as normal control group, including 7 males and 10 females as normal control group, including 9 males and 11 females. All the patients in the control group were treated in Anhui Stomatological Hospital and Anhui Provincial Hospital. Fasting venous blood was collected from patients in each group in the morning. The blood samples were collected and stored in -70 C refrigerator for 10 mins. The expression of proinflammatory factor IL-1 尾 6 and TNF- 偽 in serum of 30 patients with trigeminal neuralgia and 17 patients with malignant tumor was detected by double antibody sandwich enzyme linked immunosorbent assay (Elisa). According to the operation steps shown in the ELISA kit, the standard curve was drawn, and the corresponding levels of pro-inflammatory factors were found out on the standard curve according to the optical density of the sample. Finally, statistical analysis was carried out. Results the levels of proinflammatory factor IL-1 尾 -IL-6 and TNF- 偽 in peripheral blood of patients with trigeminal neuralgia were significantly higher than those in healthy controls (P 0.05), but there was no significant difference between the patients with trigeminal neuralgia and the patients with malignant tumor and normal inflammation. Conclusion the expression of proinflammatory factor IL-1 尾-6 and TNF- 偽 in peripheral blood of patients with trigeminal neuralgia was significantly increased, suggesting that inflammatory factors may be associated with the pathogenesis of trigeminal neuralgia.
