【摘要】：目的：前瞻性隊(duì)列研究比較改良手法復(fù)位與誘發(fā)體位訓(xùn)練法對(duì)良性發(fā)作性位置眩暈的療效。 方法：對(duì)2012年10月到2014年2月期間，在福建醫(yī)科大學(xué)附屬第二醫(yī)院神經(jīng)內(nèi)科門診及住院的120例良性發(fā)作性位置性眩暈患者按照隨機(jī)原則進(jìn)行分組，分為使用改良Epley手法治療、使用改良Epley手法+誘發(fā)體位訓(xùn)練法、使用誘發(fā)體位訓(xùn)練法3組，給予不同的治療，觀察療效。BPPV的診斷依據(jù)位置試驗(yàn)，分類主要根據(jù)誘發(fā)頭位和眼球震顫方向。 結(jié)果：120例BPPV患者中后半規(guī)管103例（管結(jié)石癥93例，嵴帽結(jié)石癥10例），，水平半規(guī)管17例（管結(jié)石癥15例，嵴帽結(jié)石癥2例），上半規(guī)管管結(jié)石癥0例。120例患者中，男性33例，女性87例，平均年齡(57±2.82)歲；后半規(guī)管眼震潛伏期(4.67±2.68)s,眼震持續(xù)時(shí)間(10.75±6.5) s；水平半規(guī)眼震潛伏期(5.32±3.33)s，眼震持續(xù)時(shí)間(10±5.86)s，兩者的眼震潛伏期和眼震持續(xù)時(shí)間比較，差異無(wú)統(tǒng)計(jì)學(xué)意義(P=0.199和P=0.147)。120例患者中使用改良Epley手法治療40例，首次治愈率82.50%；使用改良Epley手法+誘發(fā)體位訓(xùn)練法40例，首次治愈率80%；使用誘發(fā)體位訓(xùn)練法40例，首次治愈率35.00%；隨訪1周后治愈率分別為92.5%、92.5%、90%,隨訪3個(gè)月后3組治愈率分別為97.50%、100%、100%。 結(jié)論：手法復(fù)位治療良性位置性眩暈效果顯著，誘發(fā)體位訓(xùn)練法即刻治愈率稍差，但3個(gè)月治愈率與改良的Epley手法無(wú)明顯差別，且遠(yuǎn)期的復(fù)發(fā)率更低。
[Abstract]:Objective: to compare the effect of modified manual reduction and induced posture training on benign paroxysmal positional vertigo in prospective cohort study. Methods: from October 2012 to February 2014, 120 patients with benign paroxysmal positional vertigo in the Department of Neurology, second affiliated Hospital of Fujian Medical University were divided into two groups according to the random principle. The modified Epley maneuver training method was used to induce postural position, and the three groups were treated with different treatments. The diagnosis of the therapeutic effect. BPPV was based on the location test. The classification was mainly based on the induced head position and the direction of nystagmus. Results there were 103 cases of posterior semicircular canal (93 cases of tubulolithiasis, 10 cases of cristolithiasis, 17 cases of horizontal semicircular canal disease (15 cases of tubeolithiasis, 2 cases of Cristolithiasis, 0 cases of upper semicircular canal calculi, 33 cases of male). The latency of posterior semicircular canal nystagmus was 4.67 鹵2.68 s, the duration of nystagmus was 10.75 鹵6.5) s, the latency of horizontal hemiformal nystagmus was 5.32 鹵3.33 Ms, the duration of nystagmus was 10 鹵5.86 Ms, the latency of nystagmus and the duration of nystagmus were compared between the two groups. There was no significant difference between the two groups. 40 cases were treated with modified Epley maneuver, the first cure rate was 82.50, 40 cases were treated with modified Epley manipulation, the first cure rate was 80%, and the induced posture training method was used in 40 cases. The first cure rate was 35.00, the cure rate was 92.5 and 92.5 after one week follow-up, and the cure rate in the three groups was 97.50 and 100 after 3 months follow-up. Conclusion: the therapeutic effect of manual reduction on benign positional vertigo is remarkable, and the immediate cure rate of induced posture training is a little poor, but the cure rate of 3 months is not significantly different from that of modified Epley manipulation, and the long-term recurrence rate is even lower.
