【摘要】：目的探討自身免疫性腦炎的臨床特點(diǎn)以及不同的腫瘤干預(yù)方法對預(yù)后可能的影響。方法納入自身免疫性腦炎患者14例,分析其臨床癥狀、輔助檢查結(jié)果、合并腫瘤情況、治療方案,并利用改良Rankin量表(modified Rankin Scale,mRS)對其神經(jīng)功能進(jìn)行評價。結(jié)果男女比例為1:1.8,平均發(fā)病年齡38.4±18.3歲,呈急性或亞急性起病,臨床癥狀主要為癲癇、精神異常、記憶力減退及反應(yīng)遲鈍,其中3例患者合并腫瘤。大部分患者均接受了糖皮質(zhì)激素及免疫球蛋白治療,接受免疫治療后,患者的m RS較前改善(P0.001)。結(jié)論早期免疫治療對自身免疫性腦炎總體效果良好,但治療總療程目前暫不明確,如發(fā)現(xiàn)合并腫瘤,及時干預(yù)對預(yù)后可能有所幫助。
[Abstract]:Objective to investigate the clinical features of autoimmune encephalitis and the possible influence of different tumor intervention methods on prognosis. Methods 14 patients with autoimmune encephalitis were included. Their clinical symptoms, adjuvant examination results, combined tumor status, treatment plan were analyzed, and their neurological function was evaluated by modified Rankin Scalem RSs. Results the ratio of male to female was 1: 1.8. The average age of onset was 38.4 鹵18.3 years old. The main clinical symptoms were epilepsy, mental disorder, memory loss and slow reaction. Most of the patients received glucocorticoid and immunoglobulin therapy. Conclusion the overall effect of early immunotherapy on autoimmune encephalitis is good, but the total course of treatment is not clear at present. If the tumor is found, timely intervention may be helpful to the prognosis.
