【摘要】：目的探討X線引導下經(jīng)卵圓孔內(nèi)側壁進針不同深度射頻消融(RFA)治療原發(fā)性三叉神經(jīng)痛的臨床應用價值。方法選擇原發(fā)性三叉神經(jīng)痛患者32例,采用X線引導下經(jīng)卵圓孔內(nèi)側壁進針不同深度RFA治療原發(fā)性三叉神經(jīng)痛的臨床資料,觀察術中責任神經(jīng)精確復制率,術前、術后1 d、術后1周和3個月及1年的治愈率。結果術中第1支精確復制率85.7%(6/7),第2支精確復制率96.4%,第3支精確復制率100%。術后第1天治愈率為87.5%,術后1周治愈率為93.8%,術后3個月治愈率為93.8%,術后1年治愈率為87.5%。結論 X線引導下卵圓孔內(nèi)側壁進針,能準確復制責任神經(jīng)支配區(qū)疼痛癥狀,精準治療三叉神經(jīng)痛,具有微創(chuàng)、安全、療效確切等優(yōu)點,值得推廣。
[Abstract]:Objective to evaluate the clinical value of radiofrequency ablation (RFAA) in the treatment of primary trigeminal neuralgia with radiofrequency ablation through the medial wall of foramen ovale under X-ray guidance.Methods the clinical data of 32 patients with primary trigeminal neuralgia treated with RFA with different depth into the medial wall of foramen ovale were selected. The exact replication rate of responsible nerve was observed before operation.The cure rate was 1 day, 1 week and 3 months and 1 year after operation.Results the accuracy rate of the first branch was 85.7%, the second branch was 96.4%, the third branch was 100%.The cure rate was 87.5 on the first day after operation, 93.8on the first week after operation, 93.8on the third month after operation, and 87.5 at one year after operation.Conclusion X ray guided needle injection into the medial wall of foramen ovale can accurately replicate the symptoms of pain in the responsible innervation area and accurately treat trigeminal neuralgia. It has the advantages of minimally invasive, safe and effective, and is worth popularizing.
