高收縮壓軌跡是腦出血的危險(xiǎn)因素

【摘要】:目的探討不同的收縮壓軌跡對(duì)腦出血的影響。方法以參加2006-2007、2008-2009和2010-2011年3次健康體檢的開(kāi)灤研究人群作為研究隊(duì)列,其中符合入選標(biāo)準(zhǔn)的研究對(duì)象共55 194人。收縮壓軌跡通過(guò)SAS Proc Traj程序確定;采用壽命表法計(jì)算不同收縮壓軌跡組腦出血的累積發(fā)生率,并用Log-rank檢驗(yàn)比較不同收縮壓軌跡組腦出血累積發(fā)生率的差異;采用Cox比例風(fēng)險(xiǎn)模型分析不同收縮壓軌跡組對(duì)腦出血的影響。結(jié)果納入統(tǒng)計(jì)分析的研究對(duì)象55 194人通過(guò)SAS Proc Traj程序確定了5組不同的收縮壓軌跡:低-穩(wěn)定組、正常-穩(wěn)定組、中-高組、高-中組和高-穩(wěn)定組。平均隨訪(fǎng)(4.99±0.39)年,共發(fā)生腦出血164例。低-穩(wěn)定組、正常-穩(wěn)定組、中-高組、高-中組和高-穩(wěn)定組腦出血的累積發(fā)生率分別為0.05%、0.20%、0.80%、0.60%和0.80%(P0.01)。多因素Cox回歸分析顯示,在校正多種混雜因素后,與低-穩(wěn)定組相比,正常-穩(wěn)定組、中-高組、高-中組和高-穩(wěn)定組腦出血的發(fā)生風(fēng)險(xiǎn)均增加,其HR(95%CI)分別為3.48(1.41~8.56)、15.01(5.67~39.71)、11.47(3.70~35.61)和18.40(5.28~64.14)。結(jié)論高收縮壓軌跡是發(fā)生腦出血的獨(dú)立危險(xiǎn)因素。
[Abstract]:Objective to investigate the effect of different systolic blood pressure trajectories on intracerebral hemorrhage.Methods A total of 55,194 subjects were enrolled in the Kailuan study, who participated in three health checkups from 2006 to 2007, 2008-2009 and 2010-2011, among which 55,194 subjects met the inclusion criteria.The systolic blood pressure trajectory was determined by SAS Proc Traj program, the cumulative incidence of cerebral hemorrhage in different systolic blood pressure loci was calculated by life table method, and the difference of cumulative incidence rate of cerebral hemorrhage in different systolic blood pressure trajectories was compared with Log-rank test.Cox proportional risk model was used to analyze the effects of different systolic blood pressure trajectories on cerebral hemorrhage.Results SAS Proc Traj program was used to determine five different systolic blood pressure trajectories: low stable group, normal stable group, middle high group, high middle group and high stable group.The mean follow-up time was 4.99 鹵0.39 years, 164 cases of intracerebral hemorrhage occurred.The cumulative incidence of intracerebral hemorrhage in low stable group, normal stable group, middle high group, high medium group and high stable group was 0. 05% and 0. 20%, 0.80% and 0. 80% respectively.Conclusion High systolic blood pressure is an independent risk factor for intracerebral hemorrhage.
