硫辛酸干預(yù)坐骨神經(jīng)缺血再灌注損傷后的電生理變化

【摘要】:背景:硫辛酸具有由硫、碳原子所構(gòu)成的封閉環(huán)狀結(jié)構(gòu),具有強(qiáng)大的抗氧化能力,現(xiàn)已被廣泛用于氧化應(yīng)激、糖尿病性白內(nèi)障、糖尿病多發(fā)性神經(jīng)病及心血管損傷等糖尿病并發(fā)癥等疾病的預(yù)防和治療。目的:探究硫辛酸在周圍神經(jīng)缺血再灌注損傷中對(duì)周圍神經(jīng)功能的保護(hù)作用。方法:建立家兔周圍神經(jīng)缺血再灌注損傷模型,依據(jù)是否進(jìn)行再灌注分為處理組與未處理組(不進(jìn)行再灌注處理),處理組依據(jù)缺血時(shí)間1,3,6 h和再灌注前是否使用硫辛酸,于每個(gè)時(shí)間點(diǎn)又分別分為實(shí)驗(yàn)組與對(duì)照組,電鏡觀察各組實(shí)驗(yàn)動(dòng)物坐骨神經(jīng)超微結(jié)構(gòu)的變化,并使用誘發(fā)電位儀檢測(cè)坐骨神經(jīng)電生理指標(biāo)的變化。結(jié)果與結(jié)論:(1)隨著缺血時(shí)間延長(zhǎng),軸突內(nèi)空泡逐漸增多,軸突發(fā)生萎縮,軸突內(nèi)微絲結(jié)構(gòu)發(fā)生聚集并出現(xiàn)了明顯的沃勒變性,而髓鞘則出現(xiàn)了增厚紊亂以及溶解現(xiàn)象。超微結(jié)構(gòu)的破壞在缺血6 h時(shí)最為嚴(yán)重,沃勒變性及髓鞘溶解現(xiàn)象明顯增加。對(duì)比實(shí)驗(yàn)組與對(duì)照組,硫辛酸的應(yīng)用減少坐骨神經(jīng)軸突中空泡數(shù)量,減輕了軸突的萎縮、沃勒變性以及脫髓鞘現(xiàn)象的產(chǎn)生;(2)隨著缺血時(shí)間延長(zhǎng),各組實(shí)驗(yàn)動(dòng)物坐骨神經(jīng)的潛伏期顯著增加,而波幅顯著降低。潛伏期在缺血6 h達(dá)到最大值,而波幅在缺血6 h達(dá)到了最小值。對(duì)比未使用硫辛酸的對(duì)照組,實(shí)驗(yàn)組坐骨神經(jīng)潛伏期明顯下降而波幅明顯增加;(3)結(jié)果說明,硫辛酸能夠改善缺血再灌注造成的周圍神經(jīng)功能損傷。
[Abstract]:Background: lipoic acid has a closed ring structure composed of sulfur and carbon atoms and has a strong antioxidant capacity. It has been widely used in oxidative stress and diabetic cataract. Prevention and treatment of diabetic complications such as diabetic polyneuropathy and cardiovascular injury. Objective: to investigate the protective effect of lipoic acid on peripheral nerve function in peripheral nerve ischemia-reperfusion injury. A rabbit model of peripheral nerve ischemia-reperfusion injury was established. According to whether reperfusion was performed, the treatment group was divided into treatment group and untreated group (no reperfusion group). The treatment group was divided into experimental group and control group at each time point according to the time of ischemia and whether or not lipoic acid was used before reperfusion. The ultrastructure of sciatic nerve in each group was observed by electron microscope, and the changes of electrophysiological indexes of sciatic nerve were detected by evoked potentiometer. Results and conclusion: with the prolongation of ischemia time, the vacuoles in axons gradually increased and axon atrophy occurred. The microfilament structure in axon gathered and showed obvious Walller degeneration, while the myelin sheath showed thickening disorder and dissolving phenomenon. The damage of ultrastructure was the most serious at 6 h after ischemia. Compared with the control group, the application of lipoic acid reduced the number of axonal vacuoles of sciatic nerve and reduced the atrophy of axon. With the prolongation of ischemic time, the latency of sciatic nerve in each group increased significantly, but the amplitude decreased significantly, and the latency reached the maximum at 6 h after ischemia. Compared with the control group without lipoic acid, the latency of the sciatic nerve in the experimental group decreased significantly and the amplitude of the wave increased significantly. Lipoic acid can improve the injury of peripheral nerve function caused by ischemia reperfusion.
