【摘要】：目的:通過(guò)檢測(cè)5-脂氧合酶（5-lipoxygenase，5-LOX）在腦膜瘤組織中的表達(dá)情況，以探討其在腦膜瘤的發(fā)生和發(fā)展過(guò)程中的意義。方法:①實(shí)驗(yàn)用標(biāo)本來(lái)自于2012年3月至2013年1月瀘州醫(yī)學(xué)院附屬醫(yī)院神經(jīng)外科通過(guò)開(kāi)顱手術(shù)切除且由病理檢查確診的腦膜瘤組織，共65例。全部病例在本次手術(shù)前都沒(méi)進(jìn)行過(guò)規(guī)范的化療和（或）放射治療或其它相關(guān)治療，且各腦膜瘤標(biāo)本的病理資料和臨床資料（包括影像資料）均完整。對(duì)照組為正常腦膜組織（均為蛛網(wǎng)膜組織），均來(lái)自于急性重型閉合性顱腦外傷行內(nèi)外減壓術(shù)的患者，共15例。②將腦膜瘤標(biāo)本制備后按世界衛(wèi)生組織（WHO）2007年標(biāo)準(zhǔn)對(duì)其進(jìn)行病理分級(jí)并分為兩組。③所有的對(duì)照組標(biāo)本均在分離人體后12小時(shí)之內(nèi)進(jìn)行冷凍、固定處理。④對(duì)腦膜瘤及正常腦膜組織進(jìn)行免疫組化鏈霉菌抗生物素蛋白-過(guò)氧化物酶連結(jié)（S-P）法染色處理，檢測(cè)其中的5-LOX表達(dá)情況。⑤由兩名統(tǒng)計(jì)人員進(jìn)行觀察和統(tǒng)計(jì)5-LOX在腦膜瘤組織中和蛛網(wǎng)膜中的表達(dá)情況（主要是5-LOX在細(xì)胞中陽(yáng)性表達(dá)的位置和顏色以及陽(yáng)性表達(dá)細(xì)胞在所選視野中的比例等）。結(jié)果:①5-LOX在蛛網(wǎng)膜組織中表達(dá)11例陰性，4例弱陽(yáng)性；在腦膜瘤組織中的表達(dá)8例陰性，20例弱陽(yáng)性，19例陽(yáng)性，18例強(qiáng)陽(yáng)性，兩者之間差異有顯著性（2=15.886，，P=0.0000.05)。②5-LOX在Ⅰ級(jí)腦膜瘤組織表達(dá)中,8例陰性，19例弱陽(yáng)性，15例陽(yáng)性及14例強(qiáng)陽(yáng)性；在Ⅱ級(jí)腦膜瘤組織表達(dá)中，1例弱陽(yáng)性，4例陽(yáng)性和4例強(qiáng)陽(yáng)性，無(wú)陰性表達(dá)例數(shù)。二者之間差異有顯著性（2=4.353，p=0.0380.05）。③Ⅰ級(jí)腦膜瘤伴有瘤周水腫的有23例，水腫率41.2%，Ⅱ級(jí)腦膜瘤伴有瘤周水腫的有8例，水腫率88.9%，差異有顯著性（2=7.107，p=0.0080.05）。④在Ⅰ級(jí)腦膜瘤中，5-LOX的陽(yáng)性表達(dá)率為44.1%（陽(yáng)性例數(shù)和強(qiáng)陽(yáng)性例數(shù)之和占Ⅰ級(jí)腦膜瘤標(biāo)本總數(shù)的比例），與瘤周水腫呈正相關(guān)關(guān)系（2=5.731，p 0.05，=0.304）。在Ⅱ級(jí)腦膜瘤中，5-LOX的陽(yáng)性表達(dá)率為88.9%（陽(yáng)性例數(shù)和強(qiáng)陽(yáng)性例數(shù)之和占Ⅱ級(jí)腦膜瘤標(biāo)本總數(shù)的比例），與瘤周水腫呈正相關(guān)關(guān)系（2=9，p0.05，=0.707）。⑤27例男性患者中，24例為Ⅰ級(jí)，3例為Ⅱ級(jí)；38例女性患者中，32例為Ⅰ級(jí)，6例為Ⅱ級(jí)，性別與腫瘤級(jí)別之間差異無(wú)顯者性（2=0.29，p=0.590.05）。⑥27例男性患者中，11例5-LOX表達(dá)呈陰性，16例5-LOX表達(dá)呈陽(yáng)性；38例女性患者中，17例5-LOX表達(dá)呈陰性，21例5-LOX表達(dá)呈陽(yáng)性，性別與腫瘤中5-LOX的表達(dá)之間差異無(wú)顯著性（2=0.103，p=0.7490.05）。結(jié)論:①5-LOX在腦膜瘤組織中呈過(guò)度表達(dá)，推測(cè)5-LOX的過(guò)度表達(dá)可能參與了腦膜瘤的發(fā)生、發(fā)展。②5-LOX在Ⅰ、Ⅱ級(jí)腦膜瘤組織中的表達(dá)差異有顯著性，可以推測(cè)5-LOX的過(guò)度表達(dá)可能與腦膜瘤的分級(jí)之間存在聯(lián)系。③5-LOX在Ⅰ級(jí)腦膜瘤組織中的陽(yáng)性表達(dá)率低于在Ⅱ級(jí)腦膜瘤組織中的表達(dá)率，而Ⅰ級(jí)腦膜瘤瘤周水腫率也低于Ⅱ級(jí)腦膜瘤瘤周水腫率，說(shuō)明5-LOX的陽(yáng)性表達(dá)與瘤周水腫之間可能存在一定的關(guān)聯(lián)性。④性別與腦膜瘤的級(jí)別以及腫瘤中5-LOX的表達(dá)無(wú)明確關(guān)系。
[Abstract]:Objective: to detect 5- lipoxygenase (5-lipoxygenase, 5-LOX) expression in meningiomas, and to study the occurrence and development of meningioma in significance. Methods: the experimental samples were collected from March 2012 to January 2013, the Department of Neurosurgery of Affiliated Hospital of Luzhou Medical College by craniotomy and resection of the pathological diagnosis of meningioma, 65 cases. All the cases in the surgery have not been standardized chemotherapy and (or) radiotherapy or other related treatment, and pathological data and clinical data of the meningioma specimens (including image data) are complete. The control group was normal brain tissue (both arachnoid tissue), from acute severe closed craniocerebral trauma underwent decompressive craniotomy in patients with a total of 15 cases. The meningioma specimens were prepared according to the WHO (WHO) 2007 the standard pathological grading and divided into All of the two groups. The control group were in the 12 hours after the separation of body within the fixed freezing processing. Meningiomas and normal brain tissues by immunohistochemical streptavidin Perosidase (S-P) treatment staining method to detect the 5-LOX expression. The expression of the two statistics personnel of observation and statistics of 5-LOX in meningioma tissues and arachnoid in (mainly 5-LOX in cells with positive expression of color and position as well as positive cells in the selected field ratio). Results: the expression of 5-LOX in 11 cases of negative arachnoid tissue, 4 cases weakly positive; 8 cases of negative expression in meningiomas, 20 cases weakly positive, 19 positive cases, 18 cases were strongly positive, there was significant difference between them (2=15.886, P=0.0000.05).
