腦膜瘤中5-LOX的表達及其意義

【摘要】:目的:通過檢測5-脂氧合酶(5-lipoxygenase,5-LOX)在腦膜瘤組織中的表達情況,以探討其在腦膜瘤的發(fā)生和發(fā)展過程中的意義。方法:①實驗用標本來自于2012年3月至2013年1月瀘州醫(yī)學院附屬醫(yī)院神經(jīng)外科通過開顱手術切除且由病理檢查確診的腦膜瘤組織,共65例。全部病例在本次手術前都沒進行過規(guī)范的化療和(或)放射治療或其它相關治療,且各腦膜瘤標本的病理資料和臨床資料(包括影像資料)均完整。對照組為正常腦膜組織(均為蛛網(wǎng)膜組織),均來自于急性重型閉合性顱腦外傷行內(nèi)外減壓術的患者,共15例。②將腦膜瘤標本制備后按世界衛(wèi)生組織(WHO)2007年標準對其進行病理分級并分為兩組。③所有的對照組標本均在分離人體后12小時之內(nèi)進行冷凍、固定處理。④對腦膜瘤及正常腦膜組織進行免疫組化鏈霉菌抗生物素蛋白-過氧化物酶連結(jié)(S-P)法染色處理,檢測其中的5-LOX表達情況。⑤由兩名統(tǒng)計人員進行觀察和統(tǒng)計5-LOX在腦膜瘤組織中和蛛網(wǎng)膜中的表達情況(主要是5-LOX在細胞中陽性表達的位置和顏色以及陽性表達細胞在所選視野中的比例等)。結(jié)果:①5-LOX在蛛網(wǎng)膜組織中表達11例陰性,4例弱陽性;在腦膜瘤組織中的表達8例陰性,20例弱陽性,19例陽性,18例強陽性,兩者之間差異有顯著性(2=15.886,,P=0.0000.05)。②5-LOX在Ⅰ級腦膜瘤組織表達中,8例陰性,19例弱陽性,15例陽性及14例強陽性;在Ⅱ級腦膜瘤組織表達中,1例弱陽性,4例陽性和4例強陽性,無陰性表達例數(shù)。二者之間差異有顯著性(2=4.353,p=0.0380.05)。③Ⅰ級腦膜瘤伴有瘤周水腫的有23例,水腫率41.2%,Ⅱ級腦膜瘤伴有瘤周水腫的有8例,水腫率88.9%,差異有顯著性(2=7.107,p=0.0080.05)。④在Ⅰ級腦膜瘤中,5-LOX的陽性表達率為44.1%(陽性例數(shù)和強陽性例數(shù)之和占Ⅰ級腦膜瘤標本總數(shù)的比例),與瘤周水腫呈正相關關系(2=5.731,p 0.05,=0.304)。在Ⅱ級腦膜瘤中,5-LOX的陽性表達率為88.9%(陽性例數(shù)和強陽性例數(shù)之和占Ⅱ級腦膜瘤標本總數(shù)的比例),與瘤周水腫呈正相關關系(2=9,p0.05,=0.707)。⑤27例男性患者中,24例為Ⅰ級,3例為Ⅱ級;38例女性患者中,32例為Ⅰ級,6例為Ⅱ級,性別與腫瘤級別之間差異無顯者性(2=0.29,p=0.590.05)。⑥27例男性患者中,11例5-LOX表達呈陰性,16例5-LOX表達呈陽性;38例女性患者中,17例5-LOX表達呈陰性,21例5-LOX表達呈陽性,性別與腫瘤中5-LOX的表達之間差異無顯著性(2=0.103,p=0.7490.05)。結(jié)論:①5-LOX在腦膜瘤組織中呈過度表達,推測5-LOX的過度表達可能參與了腦膜瘤的發(fā)生、發(fā)展。②5-LOX在Ⅰ、Ⅱ級腦膜瘤組織中的表達差異有顯著性,可以推測5-LOX的過度表達可能與腦膜瘤的分級之間存在聯(lián)系。③5-LOX在Ⅰ級腦膜瘤組織中的陽性表達率低于在Ⅱ級腦膜瘤組織中的表達率,而Ⅰ級腦膜瘤瘤周水腫率也低于Ⅱ級腦膜瘤瘤周水腫率,說明5-LOX的陽性表達與瘤周水腫之間可能存在一定的關聯(lián)性。④性別與腦膜瘤的級別以及腫瘤中5-LOX的表達無明確關系。
[Abstract]:Objective: to detect 5- lipoxygenase (5-lipoxygenase, 5-LOX) expression in meningiomas, and to study the occurrence and development of meningioma in significance. Methods: the experimental samples were collected from March 2012 to January 2013, the Department of Neurosurgery of Affiliated Hospital of Luzhou Medical College by craniotomy and resection of the pathological diagnosis of meningioma, 65 cases. All the cases in the surgery have not been standardized chemotherapy and (or) radiotherapy or other related treatment, and pathological data and clinical data of the meningioma specimens (including image data) are complete. The control group was normal brain tissue (both arachnoid tissue), from acute severe closed craniocerebral trauma underwent decompressive craniotomy in patients with a total of 15 cases. The meningioma specimens were prepared according to the WHO (WHO) 2007 the standard pathological grading and divided into All of the two groups. The control group were in the 12 hours after the separation of body within the fixed freezing processing. Meningiomas and normal brain tissues by immunohistochemical streptavidin Perosidase (S-P) treatment staining method to detect the 5-LOX expression. The expression of the two statistics personnel of observation and statistics of 5-LOX in meningioma tissues and arachnoid in (mainly 5-LOX in cells with positive expression of color and position as well as positive cells in the selected field ratio). Results: the expression of 5-LOX in 11 cases of negative arachnoid tissue, 4 cases weakly positive; 8 cases of negative expression in meningiomas, 20 cases weakly positive, 19 positive cases, 18 cases were strongly positive, there was significant difference between them (2=15.886, P=0.0000.05).
