【摘要】：目的報(bào)道1例伴有精神癥狀的脊髓亞急性聯(lián)合變性(SCD)共病甲狀腺功能減退癥的臨床病例,并結(jié)合國內(nèi)外文獻(xiàn),總結(jié)該病的臨床特點(diǎn),提高對該病的認(rèn)識。方法報(bào)道一例廣西醫(yī)科大學(xué)第一附屬醫(yī)院收治的伴有精神癥狀的SCD共病甲減的病例,并結(jié)合文獻(xiàn)分析此病的臨床特點(diǎn)、檢查結(jié)果、治療方法和轉(zhuǎn)歸情況。結(jié)果患者中年男性,有甲亢后甲減病史,不規(guī)律服藥治療,此次發(fā)病主要表現(xiàn)為發(fā)作性心慌、視物旋轉(zhuǎn)、昏倒感,有行走不穩(wěn)、言語含糊等癥狀,檢查提示大細(xì)胞性貧血、甲減,考慮甲減所致精神障礙,予優(yōu)甲樂及對癥治療,療效不佳,漸出現(xiàn)意識清晰度下降、不能行走、排尿困難等,回顧分析病例資料并結(jié)合相關(guān)文獻(xiàn),考慮維生素B12 (VitB12)缺乏所致SCD伴精神癥狀可能,予補(bǔ)充VitB12,患者癥狀減輕,支持SCD診斷。結(jié)論以精神癥狀為首發(fā)癥狀的SCD在臨床上比較少見,容易誤診,診治延誤可造成永久性神經(jīng)功能損害,進(jìn)行全面細(xì)致的查體和血常規(guī)、血清VitB12、脊髓MRI等檢查有助于盡早明確診斷。
[Abstract]:Objective to report a case of hypothyroidism associated with subacute combined degeneration of spinal cord (SCD) with psychiatric symptoms and to summarize the clinical features of the disease. Methods A case of hypothyroidism with mental symptoms of SCD was reported in the first affiliated Hospital of Guangxi Medical University. Results the patients had a history of hyperthyroidism after hypothyroidism and irregular medication treatment. The main symptoms of the disease were paroxysmal palpitation, visual rotation, fainting sensation, unsteady walking, vague speech and so on. The examination indicated that large cell anemia, hypothyroidism, mental disorder caused by hypothyroidism, treatment with euthyroidism and symptomatic treatment, poor curative effect, decreased consciousness, inability to walk, difficulty in urination, etc. To review and analyze the case data and related literature, consider the possibility of SCD with mental symptoms caused by vitamin B12) deficiency, supplement VitB 12, reduce the symptoms of patients and support the diagnosis of SCD. Conclusion SCD with mental symptoms as the first symptom is relatively rare in clinic. It is easy to misdiagnose, delay in diagnosis and treatment can cause permanent neurological damage, comprehensive and meticulous physical examination and blood routine examination, serum VitB 12, spinal cord MRI and so on can help to make a definite diagnosis as soon as possible.
