【摘要】：目的探討缺血性卒中患者腎小球?yàn)V過(guò)率與擴(kuò)大的血管周?chē)g隙在預(yù)后評(píng)價(jià)中的價(jià)值。方法共171例急性缺血性卒中患者按照改良Rankin量表(m RS)評(píng)分分為預(yù)后不良組(88例)和預(yù)后良好組(83例),檢測(cè)血清肌酐水平并計(jì)算腎小球?yàn)V過(guò)率,計(jì)數(shù)基底節(jié)區(qū)和半卵圓中心擴(kuò)大的血管周?chē)g隙數(shù)量并評(píng)分,測(cè)量基底節(jié)區(qū)和半卵圓中心擴(kuò)大的血管周?chē)g隙直徑并分級(jí)。結(jié)果與對(duì)照組相比,預(yù)后不良組和預(yù)后良好組擴(kuò)大的血管周?chē)g隙評(píng)分(P=0.002,0.000)和血清肌酐水平(P=0.000,0.000)均升高、腎小球?yàn)V過(guò)率降低(P=0.001,0.000),預(yù)后不良組擴(kuò)大的血管周?chē)g隙評(píng)分(P=0.001)和血清肌酐水平(P=0.034)高于預(yù)后良好組、腎小球?yàn)V過(guò)率低于預(yù)后良好組(P=0.000)。擴(kuò)大的血管周?chē)g隙分級(jí)0級(jí)17例、1級(jí)89例、2級(jí)43例、3級(jí)22例,3級(jí)組和2級(jí)組血清肌酐水平高于1級(jí)組和0級(jí)組(均P=0.000)、腎小球?yàn)V過(guò)率低于1級(jí)組和0級(jí)組(均P=0.000)。相關(guān)分析顯示,預(yù)后不良組和預(yù)后良好組患者m RS評(píng)分與擴(kuò)大的血管周?chē)g隙評(píng)分呈正相關(guān)(r=0.350,P=0.033;r=0.481,P=0.036),與腎小球?yàn)V過(guò)率呈負(fù)相關(guān)(r=-0.447,P=0.018;r=-0.553,P=0.002)。結(jié)論缺血性卒中患者m RS評(píng)分與擴(kuò)大的血管周?chē)g隙評(píng)分呈正相關(guān)、與腎小球?yàn)V過(guò)率呈負(fù)相關(guān),二者均可作為缺血性卒中患者預(yù)后的預(yù)測(cè)指標(biāo)。
[Abstract]:Objective to investigate the value of glomerular filtration rate (GFR) and enlarged perivascular space in evaluating prognosis in patients with ischemic stroke. Methods 171 patients with acute ischemic stroke were divided into poor prognosis group according to modified Rankin scale / mRSs. Serum creatinine level and glomerular filtration rate were measured in 83 patients with good prognosis. Count and score the number of perivascular spaces in basal ganglia and hemi-oval center, measure the diameter and grade of perivascular space in basal ganglia and hemi-oval center. In the poor prognosis group and the good prognosis group, the enlarged perivascular space score (P0. 002 0. 000) and the serum creatinine level (P 0. 000 ~ 0. 000) were increased, the glomerular filtration rate was decreased, and the P0. 001) and serum creatinine levels in the poor prognosis group were higher than those in the good prognosis group (P 0. 001) and the serum creatinine level (P 0. 034). The glomerular filtration rate was lower than that in the prognostic group (P = 0.000). The serum creatinine level in the enlarged perivascular space grade 0, grade 0, grade 1, grade 1, grade 1, grade 89, grade 3, grade 3, grade 22, grade 3, group 2, group 2, were higher than those in the group of grade 1 and grade 0 (all P 0. 000, glomerular filtration). The overpass rate was lower than that of grade 1 group and grade 0 group (P0. 000%). Correlation analysis showed that, In the poor prognosis group and the good prognosis group, the Mrs score was positively correlated with the enlarged perivascular space score, and was negatively correlated with the glomerular filtration rate. Conclusion there was a positive correlation between the Mrs score and the enlarged perivascular space score in patients with ischemic stroke. There was a negative correlation between glomerular filtration rate and glomerular filtration rate, both of which could be used as predictors of prognosis in ischemic stroke patients.
