【摘要】：目的探討老年人群腦卒中后癲癇(PSE)發(fā)作的臨床特點(diǎn)、腦電圖表現(xiàn)及其危險因素,為癲癇的早期預(yù)防提供依據(jù)。方法選取儋州市人民醫(yī)院收治的腦卒中患者1485例,最終納入患者992例,分析患者的臨床特點(diǎn)及腦電圖表現(xiàn)。根據(jù)腦卒中后癲癇發(fā)作情況,將其分為癲癇發(fā)作組87例和無癲癇發(fā)作組905例。應(yīng)用單因素及多因素Logistic回歸分析PSE的危險因素。結(jié)果 PSE的發(fā)生率為8.77%,其中早發(fā)型癲癇占55.17%,遲發(fā)型癲癇占44.83%。PSE發(fā)作的腦電圖呈彌散性異常占26.44%,局限性異常占48.28%。單因素及多因素Logistic回歸分析顯示,高同型半胱氨酸血癥、病灶部位、病灶范圍、電解質(zhì)紊亂、日常生活能力及NIHSS評分≥25分是PSE發(fā)作的獨(dú)立危險因素,其OR(95%CI)值分別為2.063(1.146~3.125)、6.285(5.168~9.047)、4.725(3.684~6.835)、3.029(2.016~4.532)、2.538(1.568~3.562)和3.163(2.085~4.726)。結(jié)論老年人群腦卒中后癲癇發(fā)作的發(fā)生率較高,影響癲癇發(fā)作的危險因素較多,對存在危險因素的患者應(yīng)高度警惕其癲癇的發(fā)生。
[Abstract]:Objective to investigate the clinical features, electroencephalogram (EEG) manifestations and risk factors of PSEs after stroke in elderly people, and to provide evidence for early prevention of epilepsy. Methods 1485 stroke patients treated in Danzhou people's Hospital were selected. Finally, 992 patients were included. The clinical characteristics and EEG manifestations of the patients were analyzed. The risk factors of PSE were analyzed by univariate and multivariate Logistic regression analysis. Results the incidence of PSE was 8.770.The incidence of early epilepsy was 55.17, and the rate of delayed epilepsy was 44.833.PSEs were used to analyze the risk factors of Epilepsy group (n = 87) and non-epileptic seizure group (n = 905). The risk factors of PSE were analyzed by univariate and multivariate Logistic regression analysis. The electroencephalogram was diffusely abnormal in 26.44% and localized in 48.28%. Univariate and multivariate Logistic regression analysis showed that, Hyperhomocysteinemia, lesion location, focal area, electrolyte disturbance, ADL and NIHSS score 鈮，
