礦難所致PTSD患者靜息狀態(tài)下腦功能和神經(jīng)網(wǎng)絡(luò)的變化

【摘要】:實(shí)驗(yàn)一礦難所致PTSD患者靜息狀態(tài)下腦功能的變化 目的:探討單一因素、高強(qiáng)度、長(zhǎng)時(shí)間創(chuàng)傷暴露所致的PTSD患者靜息狀態(tài)下腦功能的變化。方法:采用自發(fā)性低頻振幅(ALFF)的方法研究受試患者腦功能的變化,,研究對(duì)象為15名急性PTSD患者和15名健康對(duì)照。結(jié)果:與對(duì)照組相比,PTSD組右側(cè)杏仁核、顳上回、雙側(cè)海馬旁回ALFF升高;左側(cè)蒼白球和雙側(cè)丘腦ALFF減低。差異具有統(tǒng)計(jì)學(xué)顯著性。結(jié)論:用ALFF的方法觀察了PTSD患者大腦自發(fā)低頻波動(dòng),發(fā)現(xiàn)患者邊緣系統(tǒng)部分腦區(qū)ALFF升高;雙側(cè)丘腦、左側(cè)蒼白球ALFF減低。實(shí)驗(yàn)證明了PTSD患者調(diào)控情緒中樞活動(dòng)異常,這些異?赡苁荘TSD發(fā)生發(fā)展的病理基礎(chǔ)。 實(shí)驗(yàn)二礦難所致PTSD患者靜息狀態(tài)下腦神經(jīng)網(wǎng)絡(luò)的變化 目的:探討礦難所致的PTSD患者腦神經(jīng)網(wǎng)絡(luò)的變化。方法:采用功能連接的方法研究受試者腦神經(jīng)網(wǎng)絡(luò)的變化,研究對(duì)象包括15名PTSD患者和15名健康對(duì)照。結(jié)果:PTSD組雙側(cè)島葉、左側(cè)扣帶回中部、左側(cè)中央旁小葉、右側(cè)海馬旁回、右側(cè)顳上回、右側(cè)楔葉、右側(cè)小腦與后扣帶回的功能連接較NC組顯著升高。在P0.001(未校正),體素值15Voxel的顯著性水平下未發(fā)現(xiàn)功能連接減低的結(jié)果。結(jié)論:用功能連接的方法對(duì)PTSD的致病機(jī)理進(jìn)行了探討,發(fā)現(xiàn)了PTSD患者腦內(nèi)存在大量異常功能連接,這些異?赡苁荘TSD的神經(jīng)病理機(jī)制。
[Abstract]:Changes of brain function in patients with PTSD induced by mine accident in resting state. Objective: to investigate the changes of brain function in patients with PTSD induced by single factor, high intensity and long time trauma exposure. Results: compared with the control group, ALFF in the right amygdala, superior temporal gyrus and bilateral pariprahippocampal gyrus increased in 15 patients with acute PTSD and 15 healthy controls. The difference between left globus pallidus and bilateral thalamus was statistically significant. Conclusion: the spontaneous low frequency fluctuation of brain in patients with PTSD was observed by ALFF. The decrease of ALFF in the left globus pallidus demonstrated that the abnormal regulation of emotional center activity in patients with PTSD may be the pathological basis of the pathogenesis and development of PTSD. Changes of brain neural network in PTSD patients induced by mine accident in experiment two under resting condition. Objective: to investigate the changes of brain neural network in PTSD patients caused by mine accident. Methods: the changes of brain neural networks in subjects were studied by functional connection, including 15 patients with PTSD and 15 healthy controls. Left cingulate gyrus middle, left central paracentral lobule, right parahippocampal gyrus, right superior temporal gyrus, right cuneate lobe, The functional connections between the right cerebellum and the posterior cingulate gyrus were significantly higher than those in the NC group. No decrease of functional connections was found at the level of P0.001 (uncorrected, voxel value 15 Voxel). Conclusion: the pathogenetic mechanism of PTSD was investigated by functional connectivity. A large number of abnormal functional junctions were found in the brain of PTSD patients, which may be the neuropathological mechanism of PTSD.
