【摘要】：目的分析繼發(fā)性癲癇在矢狀竇、大腦鐮旁腦膜瘤中的發(fā)生機制及危害,探討如何預(yù)防和減少腫瘤相關(guān)性癲癇的發(fā)生。方法回顧性分析自2011年3月~2016年6月我科住院手術(shù)的23例竇、鐮旁腦膜瘤患者的臨床資料,通過對比不同腫瘤特征的手術(shù)方法及繼發(fā)性癲癇的發(fā)生情況。結(jié)果全部病例均手術(shù)治療,其中腫瘤I級切除19例,II級切除4例。術(shù)前有癲癇發(fā)作7例,術(shù)后24小時癲癇發(fā)作1例,術(shù)后24小時~7天發(fā)生癲癇5例,2年內(nèi)新增癲癇病例5例,3例部分運動性失語,4例出現(xiàn)對側(cè)不同程度肢體偏癱,卡方檢驗比較得出瘤周有無水腫(p=0.007)及術(shù)前有無使用抗癲癇藥物(p=0.027)對術(shù)后繼發(fā)性癲癇的發(fā)生具有影響。結(jié)論腫瘤相關(guān)性癲癇對竇、鐮旁腦膜瘤患者術(shù)后危害極大,術(shù)前有癲癇病史的患者在圍手術(shù)期應(yīng)積極給予抗癲癇治療,對于有明顯瘤周水腫、術(shù)中腦挫傷、回流靜脈損傷等高危患者術(shù)后應(yīng)積極給予藥物預(yù)防治療。
[Abstract]:Objective to analyze the secondary epilepsy in the sagittal sinus and falx meningioma in the pathogenesis and harm, discuss how to prevent and reduce the occurrence of tumor related epilepsy. Methods a retrospective analysis from March 2011 ~2016 year in June 23 cases of sinus surgery hospitalized in our department, the clinical data of falx meningioma patients, through comparison of different surgical methods tumor characteristics and secondary epilepsy. Results all cases were treated by operation, of which I grade tumor resection in 19 cases, 4 cases of grade II resection before operation. 7 cases of epilepsy, 1 cases of epilepsy after 24 hours, 24 hours after ~7 days in 5 cases of epilepsy, 2 years new epilepsy in 5 cases, 3 cases of motor aphasia, 4 cases of contralateral hemiplegia in different degrees, the chi square test comparing peritumoral edema (p=0.007) and whether preoperative use of antiepileptic drugs (p=0.027) on postoperative epilepsy had influence Conclusion tumor related epilepsy of sinus falx meningioma patients with great harm, preoperative history of epilepsy should give positive antiepileptic therapy in the perioperative period, with obvious peritumoral edema, cerebral contusion, vein injury and other high-risk patients should receive active drug prevention and treatment.
