兒童腦動靜脈畸形位置與癲癇類型和預(yù)后的相關(guān)性分析

【摘要】:目的:探討兒童腦動靜脈畸形位置與癲癇類型及癲癇預(yù)后的關(guān)系。資料與方法:收集2013.1-2015.12期間在首都醫(yī)科大學(xué)附屬北京天壇醫(yī)院的兒童腦動靜脈畸形患者病灶位置,癲癇類型,隨訪根據(jù)Engle分級評估患者癲癇預(yù)后。結(jié)果:共分析了125名入組的兒童腦動靜脈畸形患者的病例信息,其中男性患者75人,女性患者50人。平均年齡12.57歲。病變位于額葉(n=61,40.9%),顳葉(n=27,18.1%),頂葉(n=24,16.1%),枕葉(n=24,16.1%),底節(jié)區(qū)(n=2,1.3%),胼胝體(n=5,3.4%),5個位于小腦(n=5,3.4%)。其中首發(fā)癥狀為癲癇的患者共有31人(24.8%),癲癇控制預(yù)后較差的有8人(9.4%)。結(jié)論:兒童腦動靜脈畸形癲癇發(fā)生率與成人相似,位于額葉和顳葉的病灶相對于其他腦區(qū),更易發(fā)生癲癇。兒童腦動靜脈畸形位置與首發(fā)癲癇類型以及癲癇預(yù)后無明顯相關(guān)。
[Abstract]:Objective: to investigate the relationship between the location of cerebral arteriovenous malformation (AVM) in children and the type of epilepsy and the prognosis of epilepsy. Data and methods: the location and type of epilepsy in children with cerebral arteriovenous malformation (AVM) in Beijing Temple of Heaven Hospital affiliated to Capital Medical University during the period from March 1 to May 12, 2011 were collected. Results: a total of 125 children with cerebral arteriovenous malformation (AVM) were evaluated according to the Engle grade. Results: a total of 75 male patients were included in the study. 50 female patients with an average age of 12.57 years. The lesions were located in the frontal lobe of 61D, in the temporal lobe, in the frontal lobe, in the temporal lobe, in the temporal lobe, in the temporal lobe, in the temporal lobe, in the temporal lobe, in the parietal lobe, in the parietal lobe, in the parietal lobe, in the parietal lobe, in the occipital lobe, in the occipital lobe, in the occipital lobe, in the nongma 21.3, in the basal ganglia, in the callosal area, and in the corpus callosum. Five patients were located in the cerebellar gyrus 53.40.There were 31 patients with the first symptom of epilepsy, who had a poor prognosis. Conclusion: the incidence of epilepsy in children with cerebral arteriovenous malformation is similar to that in adults. The lesions located in the frontal lobe and temporal lobe were more prone to epilepsy than those in other brain regions. The location of cerebral arteriovenous malformations in children had no significant correlation with the type of initial epilepsy and the prognosis of epilepsy.
