【摘要】：目的：探討經(jīng)額外側(cè)入路切除大型嗅溝腦膜瘤的手術(shù)方法及治療效果,并與傳統(tǒng)的雙側(cè)額下入路進(jìn)行對比。方法：回顧性分析我院神經(jīng)外科自2009年3月至2013年5月經(jīng)雙側(cè)額下入路和2013年6月至今經(jīng)額外側(cè)入路顯微外科手術(shù)切除的36例大型嗅溝腦膜瘤患者的臨床資料,并進(jìn)行療效評價(jià)。結(jié)果：兩種入路在腫瘤全切率、精神癥狀的改善及術(shù)后腫瘤復(fù)發(fā)率方面并無明顯差異；雙側(cè)額下入路對于改善患者嗅覺異常優(yōu)于額外側(cè)入路；額外側(cè)入路對于改善患者視覺異常優(yōu)于雙側(cè)額下入路,在有效避免術(shù)中水腫腦組織膨出、充分顯露腫瘤、減少腦組織損傷方面,額外側(cè)入路優(yōu)于傳統(tǒng)的雙側(cè)額下入路。結(jié)論：經(jīng)額外側(cè)入路切除大型嗅溝腦膜瘤符合微侵襲手術(shù)理念,并發(fā)癥發(fā)生率相對較低,手術(shù)效果良好,可以作為大型嗅溝腦膜瘤手術(shù)切除的一種選擇術(shù)式。
[Abstract]:Objective: to investigate the surgical methods and therapeutic effect of extra lateral approach for the resection of large olfactory sulcus meningiomas. And compared with the traditional bilateral subfrontal approach. Methods:. A retrospective analysis of 36 cases of large olfactory sulcus meningioma treated by bilateral subfrontal approach from March 2009 to 2013 and microsurgery via additional approach from June 2013 to present in our hospital. Clinical data. Results: there was no significant difference between the two approaches in the total resection rate, the improvement of mental symptoms and the recurrence rate of tumor after operation. Bilateral subfrontal approach was superior to extra lateral approach in improving olfactory abnormality. The extra lateral approach is superior to bilateral subfrontal approach in improving the visual abnormality of patients. It can effectively avoid edema of brain tissue during operation, fully expose tumor, and reduce brain tissue injury. Conclusion: the excision of large olfactory sulcus meningiomas via extra lateral approach is in accordance with the concept of micro-invasive surgery, the incidence of complications is relatively low, and the surgical effect is good. Can be used as a large olfactory sulcus meningioma surgical resection of choice.
