西寧地區(qū)急性缺血性腦卒中氧化低密度脂蛋白與神經(jīng)功能缺損程度的相關(guān)性研究

【摘要】:目的:本實(shí)驗(yàn)研究西寧地區(qū)急性缺血性腦卒中患者血漿氧化低密度脂蛋白(Oxidized low density lipoprotein OXLDL)與神經(jīng)功能缺損程度的關(guān)系,為急性缺血性腦卒中患者的病情判斷、估計(jì)預(yù)后、危險(xiǎn)預(yù)測(cè)提供依據(jù)。方法:收集并整理2013年3月至2014年3月青海省人民醫(yī)院神經(jīng)內(nèi)科收住入院的漢族急性腦梗死病人120例。按照美國(guó)國(guó)立衛(wèi)生院制定的神經(jīng)功能缺損評(píng)分(NIHSS評(píng)分)對(duì)患者進(jìn)行神經(jīng)缺損評(píng)估,根據(jù)神經(jīng)功能缺損程度評(píng)分分為3組:≤4分為輕度,4分且≤25分為中度,25分為重度,其中輕度58例,年齡(62.21±9.75)歲;中度43例,年齡(63.41±11.94)歲;重度19例,年齡(60.68±11.59)歲。血漿OXLDL水平采用ELISA法檢測(cè)。采用單因素方差分析方法及pearson相關(guān)分析對(duì)各組間比較,研究OXLDL與神經(jīng)功能缺損程度的相關(guān)性。根據(jù)MRI梗死面積大小按照admas分類法分為三組,采用單因素方差分析方法及pearson相關(guān)分析對(duì)各組間比較,研究OXLDL與梗死面積的相關(guān)性。結(jié)果:1.輕度神經(jīng)功能缺損、中度神經(jīng)功能缺損與重度神經(jīng)功能缺損患者各組間一般資料比較,無(wú)統(tǒng)計(jì)學(xué)意義(P0.05);2.輕、中、重度神經(jīng)功能缺損組間OXLDL水平比較有明顯差異,有統(tǒng)計(jì)學(xué)意義(P0.05);3.相關(guān)性分析示:神經(jīng)功能缺損評(píng)分與OXLDL水平相關(guān)性分析呈正相關(guān)性(R=0.474,P0.05);4.不同腦梗死面積患者之間的氧化低密度脂蛋白水平比較,差異有統(tǒng)計(jì)學(xué)意義(P0.05);5.相關(guān)性分析示:腦梗死面積與OXLDL水平呈正相關(guān)性(R=0.647,P0.05)。結(jié)論:西寧地區(qū)急性缺血性腦卒中患者氧化低密度脂蛋白含量變化與神經(jīng)功能缺損程度存在正相關(guān),并且與腦梗死面積存在正相關(guān),揭示OXLDL為腦梗死危險(xiǎn)因素之一,在評(píng)估急性腦梗塞的病情及預(yù)后方面有一定的臨床意義。
[Abstract]:Objective: This study of Xining area in patients with acute ischemic stroke of oxidized low density lipoprotein (Oxidized low density lipoprotein OXLDL) and the relationship between the degree of neurological impairment, judgment, for patients with acute ischemic stroke disease provides evidence for the estimation of prognosis and risk prediction. Methods: 120 patients with acute cerebral infarction were collected and collected from March 2013 to March 2014 in the Department of Neurology of Qinghai People's Hospital. According to the score of nerve function defect formulated by the National Institutes of Health (NIHSS score) for nerve defect assessment for patients, according to the degree of neurological deficit score were divided into 3 groups: less than or equal to 4 divided into mild, 4 and less than 25 for moderate, 25 as severe, including 58 mild cases, age (62.21 + 9.75) at the age of 43; moderate cases, age (63.41 + 11.94) years; 19 severe cases, age (60.68 + 11.59) years old. The level of plasma OXLDL was detected by ELISA. The correlation between OXLDL and the degree of nerve function defect was studied by single factor analysis of variance and Pearson correlation analysis. According to the size of MRI infarct size, the rats were divided into three groups according to ADMAS classification. The correlation between OXLDL and infarct size was compared between the groups by one-way ANOVA and Pearson correlation analysis. Results: 1. mild neurological deficit, moderate neurological deficit and severe neurological deficits in patients with inter group comparison of general information, no statistical significance (P0.05); 2. light, moderate and severe neurological impairment group OXLDL level had significant difference compared with statistical significance (P0.05, 3.); correlation analysis showed: the correlation of neural function defect score and OXLDL level analysis showed positive correlation (R=0.474, P0.05); comparison between the 4. different area of cerebral infarction in patients with oxidized low density lipoprotein level, the difference was statistically significant (P0.05); 5. correlation analysis showed: the cerebral infarction area was correlated with OXLDL levels (R=0.647, P0.05). Conclusion: there is a positive correlation in Xining area changes in patients with acute ischemic stroke oxidized low density lipoprotein content and the degree of neurological deficit, and there is a positive correlation with the area of cerebral infarction, revealed OXLDL as one of the risk factors of cerebral infarction, has certain clinical significance in acute cerebral infarction and prognosis of the disease.
