【摘要】：目的探討全身麻醉在不同年齡輕中度上呼吸道感染(Upper respiratory infection,URI)患兒應(yīng)用中的安全性,為臨床治療提高參考依據(jù)。方法于2010年2月-2013年9月手術(shù)室安排179例擇期手術(shù)患兒,依據(jù)URI程度分級將患兒分為3組,分別為無URI組102例、輕度URI組49例、中度URI組28例,比較3組患兒麻醉期間呼吸系統(tǒng)不良事件發(fā)生率及臨床指標,數(shù)據(jù)采用SPSS 13.0軟件進行統(tǒng)計分析。結(jié)果年齡≤3歲患兒無URI組、輕度URI組與中度URI組在咳嗽、屏氣比較,中度URI組發(fā)生率明顯增高,差異有統(tǒng)計學意義(P0.05);4~12歲患兒各組之間比較,差異無統(tǒng)計學意義;各組均未出現(xiàn)喉痙攣、支氣管痙攣以及發(fā)熱;年齡≤3歲、4~12歲患兒術(shù)中心率比較,中度URI組高于無URI組,差異有統(tǒng)計學意義(P0.05);年齡≤3歲患兒術(shù)中心率比較中度URI組高于輕度URI組,差異有統(tǒng)計學意義(P0.05)。結(jié)論年齡≤3歲合并輕度URI患兒,年齡4~12歲的合并輕中度URI患兒實施氣管插管全身麻醉相對安全,而年齡≤3歲合并中度URI患兒進行氣管插管全身麻醉時并發(fā)癥率較高,存在一定風險,因此對于此類患兒則應(yīng)進行更全面風險評估,把握主次,慎重選擇是否實施全身麻醉。
[Abstract]:Objective to investigate the safety of general anesthesia in children with mild to moderate upper respiratory tract infection (Upper respiratory infection,URI) of different ages, so as to improve the reference for clinical treatment. Methods from February 2010 to September 2013, 179 children undergoing elective operation in operating room were divided into three groups according to URI grade: no URI group (n = 102), mild URI group (n = 49), moderate URI group (n = 28), non-URI group (n = 102) and moderate URI group (n = 28). The incidence of respiratory adverse events and clinical indexes were compared among the three groups during anesthesia. The data were statistically analyzed by SPSS 13.0 software. Results the incidence of cough and breath-holding in mild URI group and moderate URI group was significantly higher than that in moderate URI group (P 0.05), while there was no significant difference in the incidence of cough and breath-holding between mild URI group and moderate URI group (P0.05), while there was no significant difference between 4 and 12-year-old children with URI (P0.05).
