丙泊酚聯合瑞芬太尼靶控輸注對患者血流動力學及腦電雙頻指數的影響

【摘要】:目的探討丙泊酚聯合瑞芬太尼靶控輸注對患者血流動力學及腦電雙頻指數(BIS)的影響。方法選取2013年11月至2014年11月該院行丙泊酚聯合瑞芬太尼靶控輸注麻醉患者90例,根據瑞芬太尼靶控輸注的濃度將患者分為Ⅰ組(2.0 ng/ml)、Ⅱ組(3.0 ng/ml)和Ⅲ組(4.0 ng/ml),每組30例,三組丙泊酚的靶濃度均為4μg/ml。比較三組入室后5 min(T0)、插管前(T1)、插管后1 min(T2)、插管后3 min(T3)、插管后5 min(T4)時的心率(HR)、平均動脈壓(MAP)以及BIS,并比較蘇醒時間。結果 T2、T3、T4時刻Ⅲ組的BIS值顯著低于Ⅰ組和Ⅱ組(P0.05);Ⅰ組在T2、T3時刻的HR、MAP均顯著高于T1時刻,Ⅱ組在T2時刻HR、MAP顯著高于T1時刻(P0.05);Ⅲ組蘇醒時間顯著長于Ⅰ組和Ⅱ組(P0.05)。結論丙泊酚聯合瑞芬太尼靶控輸注麻醉隨著瑞芬太尼濃度的增加BIS值會降低,并且對血流動力學反應抑制作用也會增強。
