SD大鼠股骨骨折合并脊髓損傷的實(shí)驗(yàn)性研究

【摘要】:目的:建立一種既能成功維持長時間存活,并符合臨床特征,又簡便易行的實(shí)驗(yàn)動物模型。方法:將48只SD雄性大鼠隨即分為單純股骨骨折組(A組,24只)、股骨骨折合并脊髓損傷組(B組,24只),股骨骨折模型通過雙側(cè)小切口開放截骨術(shù)造成股骨中端橫行骨折并植入內(nèi)固定,參考Allen法,使用自制打擊裝置造成大鼠T10段脊髓急性挫傷性損傷,將骨折模型和脊髓損傷模型兩種方法融合制造股骨骨折合并脊髓損傷模型,分別在相同條件下飼養(yǎng)4周,對兩組SD大鼠在制模成功后不同時間點(diǎn)進(jìn)行大體觀察、4周時觀察骨折斷端愈合情況,觀察其并發(fā)癥,并計(jì)算造模成功率。結(jié)果與結(jié)論:術(shù)后骨折合并脊髓損傷動物模型均成活,雙下肢感覺及運(yùn)動功能喪失,但可利用雙上肢緩慢匍匐向前移動,術(shù)后前3天進(jìn)食少,活動少,夜間采用尾端懸吊后,骨折患肢未出現(xiàn)缺血壞死,到達(dá)第四周時,A組除麻醉死亡一只外均成活,B組共死亡4只,成活率為83.33%,髓腔內(nèi)固定未見脫出,此時A、B組骨折斷端均有連續(xù)骨痂生成,從體積上來看,B組骨痂組織明顯大于A組。證實(shí)將改良Allen法與小切口股骨外側(cè)開放截骨法結(jié)合后,簡單易行,可以成功制作股骨骨折合并脊髓損傷動物模型,采用術(shù)后特殊護(hù)理,可以成功讓該模型成活至第4周。
[Abstract]:Objective: to establish an experimental animal model which can successfully maintain long-time survival, accord with clinical characteristics and be easy to carry out. Methods: Forty-eight male SD rats were randomly divided into group A (n = 24) and group B (n = 24). The fracture model of femur was made by open bilateral small incision osteotomy and internal fixation was implanted. Referring to Allen method, the acute contusion injury of T10 segment spinal cord in rats was caused by self-made hitting device. The fracture model and spinal cord injury model were fused to make the femur fracture combined with spinal cord injury model. The two groups of SD rats were fed under the same conditions for 4 weeks. The two groups of SD rats were observed at different time points after the model making. At 4 weeks, the healing of fracture end was observed, the complications were observed, and the success rate of modeling was calculated. Results and conclusion: all the animal models of fracture complicated with spinal cord injury survived, the sensory and motor functions of both lower limbs were lost, but the upper limbs could be slowly creeping forward, the food intake was less and the activity was less in the first three days after operation, and the tail was suspended at night. There was no ischemic necrosis in the fractured limbs. At the end of the week, all but one of the injured limbs survived in group A, 4 in group B, with a survival rate of 83.33, and no prolapse of intramedullary fixation. Continuous callus formation was found at the fracture end in group B, and the callus tissue in group B was significantly larger than that in group A in volume. It was proved that the combination of the modified Allen method and the small incision open osteotomy of the lateral femur was simple and easy, and the animal model of femoral fracture with spinal cord injury could be made successfully. The special nursing care after operation could successfully keep the model alive until the 4th week.
