【摘要】：目的:探討視可尼可視喉鏡(SOS)在張口困難患者氣管插管全身麻醉中的應(yīng)用價(jià)值。方法:收集筆者所在醫(yī)院2015年1月-2016年1月擬行氣管插管全身麻醉患者100例,然后將100例患者隨機(jī)分為觀察組和對(duì)照組,觀察組患者50例,采用視可尼可視喉鏡行氣管插管,對(duì)照組患者50例,采用Macintoch直接喉鏡行氣管插管。然后對(duì)兩組患者的氣管插管總耗時(shí)、一次插管成功率和插管相關(guān)并發(fā)癥進(jìn)行對(duì)比和分析。結(jié)果:觀察組患者采用視可尼可視喉鏡行氣管插管的總耗時(shí)明顯短于對(duì)照組患者采用Macintoch直接喉鏡行氣管插管的總耗時(shí),差異有統(tǒng)計(jì)學(xué)意義(P0.05)。觀察組患者采用視可尼可視喉鏡行氣管插管的一次插管成功率明顯高于對(duì)照組患者采用Macintoch直接喉鏡行氣管插管的一次插管成功率,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。觀察組采用視可尼可視喉鏡行氣管插管發(fā)生氣道黏膜損傷、牙齒脫落或者松動(dòng)以及循環(huán)波動(dòng)超過基礎(chǔ)值的20%等并發(fā)癥的患者明顯少于對(duì)照組采用Macintoch直接喉鏡行氣管插管發(fā)生氣道黏膜損傷、牙齒脫落或者松動(dòng)以及循環(huán)波動(dòng)超過基礎(chǔ)值的20%等并發(fā)癥的患者,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:視可尼可視喉鏡用于張口困難患者氣管插管總耗時(shí)短,一次插管成功率高,插管相關(guān)并發(fā)癥少,值得臨床推廣運(yùn)用。
[Abstract]:Objective: to investigate the value of visual laryngoscope (SOS) in tracheal intubation general anesthesia in patients with difficult mouth opening. Methods: from January 2015 to January 2016, 100 patients with tracheal intubation were randomly divided into two groups: observation group (n = 50) and control group (n = 50). 50 patients in the control group were intubated with Macintoch direct laryngoscope. Then, the total time consuming of tracheal intubation, the success rate of one intubation and the complications associated with intubation were compared and analyzed between the two groups. Results: the total time consuming of tracheal intubation by visual laryngoscope in the observation group was significantly shorter than that in the control group by Macintoch direct laryngoscope (P0.05). The success rate of tracheal intubation with visual laryngoscope in the observation group was significantly higher than that in the control group with Macintoch direct laryngoscope (P0.05). In the observation group, tracheal intubation with visual laryngoscope was used to induce airway mucosal injury. The number of patients with complications such as tooth loss or loosening and circulation fluctuating more than 20% of the basic value was significantly less than that of the control group who underwent Macintoch direct laryngoscope to intubate airway mucous membrane injury. The difference was statistically significant in patients with tooth loss or loosening as well as circulation fluctuation exceeding 20% of the basic value (P0.05). Conclusion: the tracheal intubation by visual laryngoscope in patients with difficult mouth opening takes less time, higher success rate and less complications, so it is worth popularizing in clinic.
