【摘要】：目的探討喉罩在老年腹腔鏡膽囊切除術(shù)患者全身麻醉中的應(yīng)用效果。方法將134例老年腹腔鏡下膽囊切除術(shù)患者隨機(jī)分為觀察組與對(duì)照組,每組各67例。觀察組患者采用喉罩機(jī)械通氣,對(duì)照組患者采用氣管插管機(jī)械通氣。比較兩組患者麻醉效果、血流動(dòng)力學(xué)變化及圍術(shù)期并發(fā)癥發(fā)生率。結(jié)果兩組患者手術(shù)順利,術(shù)中麻醉滿意,術(shù)后蘇醒及時(shí),狀態(tài)良好。T0時(shí)間點(diǎn)兩組患者收縮壓(SBP)、舒張壓(DBP)、心率(HR)及脈搏血氧飽和度(Sp O2)比較差異均無(wú)顯著性(P0.05)。與T0時(shí)間點(diǎn)比較,對(duì)照組患者T1時(shí)間點(diǎn)SBP、DBP及HR明顯升高(P0.01),SBP與HR在T2時(shí)間點(diǎn)顯著降低(P0.01);觀察組患者T1時(shí)間點(diǎn)僅DBP明顯升高,且升高水平顯著低于對(duì)照組(P0.01);T_1、T_4時(shí)間點(diǎn)觀察組患者SBP、DBP及HR均顯著低于對(duì)照組(P0.01)。兩組患者5個(gè)時(shí)間點(diǎn)Sp O_2均未發(fā)生顯著變化(P0.05)。觀察組患者圍術(shù)期并發(fā)癥發(fā)生率均顯著低于對(duì)照組(P0.05或P0.01)。結(jié)論喉罩在老年腹腔鏡膽囊切除術(shù)患者全身麻醉中保證良好通氣及麻醉狀態(tài)的同時(shí),使血流動(dòng)力學(xué)更加穩(wěn)定,咽喉部并發(fā)癥明顯減少,安全性較高,值得推廣。
[Abstract]:Objective to investigate the effect of laryngeal mask on general anesthesia in elderly patients undergoing laparoscopic cholecystectomy. Methods 134 elderly patients undergoing laparoscopic cholecystectomy were randomly divided into observation group (n = 67) and control group (n = 67). The patients in the observation group were ventilated with laryngeal mask, and the patients in the control group were mechanically ventilated with tracheal intubation. Anaesthesia effect, hemodynamic changes and perioperative complications were compared between the two groups. Results the operation was smooth, the anesthesia was satisfactory, the postoperative recovery was timely and the condition was good. Systolic (SBP), diastolic blood pressure (DBP),) in both groups at T0 time point There was no significant difference in heart rate (HR) and pulse oxygen saturation (Sp O 2) (P0.05). Compared with T0 time point, the SBP,DBP and HR of the control group increased significantly at T1 time point (P0.01), SBP and HR decreased significantly at T2 time point (P0.01); In the observation group, only DBP increased significantly at T1 time point, and the level of elevation was significantly lower than that in the control group (P0.01), while SBP,DBP and HR in the observation group were significantly lower than those in the control group (P0.01). There was no significant change in Sp O\ -2 at 5 time points in both groups (P 0.05). The incidence of perioperative complications in the observation group was significantly lower than that in the control group (P0.05 or P0.01). Conclusion the laryngeal mask can ensure good ventilation and anaesthesia in the elderly patients undergoing laparoscopic cholecystectomy, at the same time, the hemodynamics is more stable, the complications of pharynx and larynx are obviously reduced, and the safety is high, which is worth popularizing.
