【摘要】：目的探討七氟烷吸入麻醉對(duì)下肢骨折手術(shù)患兒血?dú)庵笜?biāo)及麻醉質(zhì)量的影響。方法選擇2013年10月至2015年6月在本院小兒骨科行手術(shù)治療的72例下肢骨折患兒,采用隨機(jī)數(shù)字表法分為觀察組和對(duì)照組,每組36例。對(duì)照組采用氯胺酮復(fù)合骶管阻滯麻醉,觀察組給予七氟烷復(fù)合骶麻,比較兩組患兒麻醉中血?dú)庵笜?biāo)、麻醉質(zhì)量及不良反應(yīng)的發(fā)生情況。結(jié)果血?dú)庵笜?biāo):麻醉后兩組患兒PaO_2均有所下降,PaCO_2均有所升高;切皮及置入內(nèi)固定時(shí),觀察組Pa O2高于對(duì)照組,PaCO_2低于對(duì)照組,兩組比較差異均有統(tǒng)計(jì)學(xué)意義(P均0.05);麻醉質(zhì)量:兩組患兒麻醉完成時(shí)間、感覺(jué)阻滯起效時(shí)間、感覺(jué)阻滯維持時(shí)間、運(yùn)動(dòng)阻滯起效時(shí)間及運(yùn)動(dòng)阻滯維持時(shí)間比較差異均無(wú)統(tǒng)計(jì)學(xué)意義(P均0.05);麻醉蘇醒時(shí)間及不良反應(yīng):觀察組疼痛消失時(shí)間、麻醉蘇醒時(shí)間明顯短于對(duì)照組,不良反應(yīng)明顯低于對(duì)照組,兩組比較差異均有統(tǒng)計(jì)學(xué)意義(P均0.05)。結(jié)論七氟烷吸入麻醉能維持下肢骨折手術(shù)患兒血?dú)庵笜?biāo)穩(wěn)定,提高麻醉質(zhì)量,減少不良反應(yīng)。
[Abstract]:Objective to investigate the effect of sevoflurane inhalation anesthesia on blood gas index and anesthesia quality in children with lower limb fracture. Methods from October 2013 to June 2015, 72 children with lower extremity fractures were divided into observation group (n = 36) and control group (n = 36). The control group was treated with ketamine combined with sacral canal block, the observation group was given sevoflurane combined with sacral anesthesia, and the blood gas index, anesthesia quality and adverse reactions were compared between the two groups. Results Blood gas index: after anesthesia, PaO_2 in both groups decreased and Paco _ 2 increased, Pao _ 2 in the observation group was higher than that in the control group, and Paco _ 2 in the observation group was lower than that in the control group. The quality of anesthesia: the time of completion of anesthesia, the onset time of sensory block, the duration of sensory block, and the maintenance time of sensory block in the two groups were statistically significant (P 0.05). There was no significant difference in the onset time of motor block and the maintenance time of motor block (P 0.05), the time of anaesthesia recovery and side effects: the time of pain disappearance in the observation group was significantly shorter than that in the control group. The adverse reaction was significantly lower than that in the control group, and the difference between the two groups was statistically significant (P 0.05). Conclusion sevoflurane inhalation anesthesia can maintain stable blood gas index, improve anesthesia quality and reduce adverse reactions in children with lower extremity fracture.
