【摘要】：目的分析丙泊酚用于吸毒患者胃鏡檢查的現(xiàn)狀。方法選取2011年11月-2017年6月該院消化科門(mén)診及急診收治的需接受胃鏡檢查的吸毒吞食異物患者42例,隨機(jī)分為觀察組(丙泊酚麻醉)和對(duì)照組(非麻醉),觀察兩組患者循環(huán)及呼吸系統(tǒng)相關(guān)各項(xiàng)指標(biāo),對(duì)比兩組患者手術(shù)時(shí)間、出現(xiàn)插管失敗需拔管情況,以及與手術(shù)操作、麻醉用藥相關(guān)的不良反應(yīng)、手術(shù)耐受情況。結(jié)果觀察組患者的耐受率為90.48%,高于對(duì)照組的4.76%(P0.05)。觀察組患者心率及呼吸頻率在T1、T2和T3時(shí)間的波動(dòng)幅度小于對(duì)照組。觀察組患者胃鏡手術(shù)操作時(shí)間為(5.36±2.02)min,對(duì)照組操作時(shí)間為(8.84±1.97)min,觀察組患者手術(shù)時(shí)較短(P0.05);觀察組患者拔管率為0.00%,低于對(duì)照組的28.57%(P0.05)。兩組患者的高血壓、低血壓、心動(dòng)過(guò)速、心動(dòng)過(guò)緩、呼吸抑制發(fā)生率比較,差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05),觀察組患者戒斷反應(yīng)發(fā)生率低于對(duì)照組(P0.05)。結(jié)論丙泊酚用于吸毒患者的胃鏡麻醉效果較好,麻醉過(guò)程中患者自覺(jué)不適感低,耐受性佳,安全性有保障,值得臨床推廣應(yīng)用。
[Abstract]:Objective to analyze the application of propofol in gastroscopy of drug users. Methods from November 2011 to June 2017, 42 patients who needed gastroscopy to swallow foreign bodies were selected. The patients were randomly divided into observation group (propofol anesthesia) and control group (non-anesthesia group). The indexes of circulatory and respiratory system were observed and compared between the two groups. Adverse reactions associated with anesthetic use, surgical tolerance. Results the tolerance rate of the patients in the observation group was 90.48, higher than that in the control group. The changes of heart rate and respiratory frequency in T _ 1 T _ 2 and T _ 3 were lower in the observation group than in the control group. The operative time of gastroscopy in the observation group was 5.36 鹵2.02min, and that in the control group was 8.84 鹵1.97min, and that in the observation group was shorter than that in the control group (P 0.05), and the extubation rate of the patients in the observation group was 0.005g, which was lower than that in the control group (28.57g / P 0.05). There was no significant difference in the incidence of hypertension, hypotension, tachycardia, bradycardia and respiratory depression between the two groups. The rate of withdrawal reaction in the observation group was lower than that in the control group (P 0.05). Conclusion propofol is effective in gastroscopic anesthesia for drug addicts. The patients have low feeling of discomfort, good tolerance and safety during anesthesia, which is worthy of clinical application.
