【摘要】：目的觀察抗血管內(nèi)皮生長因子(vascular endothelial growth factor,VEGF)治療不同區(qū)域早產(chǎn)兒視網(wǎng)膜病變的臨床效果。方法回顧性分析2013年4月至2014年4月在鄭州兒童醫(yī)院行玻璃體內(nèi)注射抗VEGF藥物的早產(chǎn)兒視網(wǎng)膜病變患兒24例(46眼),分為Ⅰ區(qū)病變組15例(28眼),Ⅱ區(qū)病變組9例(18眼)。所有患者均在全身麻醉下行玻璃體內(nèi)注射抗VEGF藥物雷珠單抗,對患兒術(shù)后眼底進(jìn)展情況進(jìn)行觀察,分析兩組首次注射治愈和進(jìn)展情況以及并發(fā)癥發(fā)生情況。結(jié)果兩組間的出生孕周、出生體質(zhì)量相比,差異均無統(tǒng)計學(xué)意義(均為P0.05);手術(shù)矯正孕周相比,差異有統(tǒng)計學(xué)意義(P=0.001)。24例(46眼)中,首次注射治愈21眼,病情繼續(xù)發(fā)展25眼,治愈率為45.65%,治療到治愈的時間間隔為2~12個月。Ⅰ區(qū)病變組首次注射治愈9眼,治愈率為32.14%,進(jìn)展率為67.86%;Ⅱ區(qū)病變18眼中12眼首次注射治愈,6眼病情繼續(xù)進(jìn)展,治愈率為66.67%,進(jìn)展率為33.34%。兩組首次注射治愈率相比,差異有統(tǒng)計學(xué)意義(χ~2=5.263,P=0.022),Ⅰ區(qū)病變的首次注射治愈率明顯低于Ⅱ區(qū)病變。兩組進(jìn)展率相比,差異有統(tǒng)計學(xué)意義(χ~2=-2.269,P=0.023),Ⅰ區(qū)病變的進(jìn)展率明顯高于Ⅱ區(qū)病變。隨訪中未發(fā)現(xiàn)與玻璃體內(nèi)注射有關(guān)的白內(nèi)障、眼內(nèi)炎、視網(wǎng)膜裂孔等出現(xiàn),僅有2例出現(xiàn)角膜水腫,4例出現(xiàn)結(jié)膜下出血,均未做特殊處理自愈。結(jié)論玻璃體內(nèi)注射抗VEGF藥物對于Ⅰ區(qū)、Ⅱ區(qū)病變均有效,Ⅰ區(qū)病變手術(shù)成功率低,再次手術(shù)的幾率大,但可為再次手術(shù)創(chuàng)造時間和條件。
[Abstract]:Objective to observe the clinical effect of vascular endothelial growth factor (VEGF) on retinopathy of premature infants in different regions.Methods from April 2013 to April 2014, 24 children with retinopathy of preterm infants who received intravitreous injection of anti- VEGF drugs in Zhengzhou Children's Hospital were retrospectively analyzed. The patients were divided into 15 patients (28 eyes) and 9 patients (18 eyes).All patients were given intravitreous injection of anti-#en0# drug Lei Zhu McAb under general anesthesia to observe the fundus progress after operation and to analyze the first injection of cure and progression and the occurrence of complications in the two groups.Results there was no significant difference in birth weight and birth weight between the two groups (all P0.05A, and the difference was statistically significant in 24 patients (46 eyes).) 21 eyes were cured by the first injection and 25 eyes continued to develop.The cure rate was 45.65 and the interval between treatment and cure was 2 ~ 12 months.
