【摘要】：目的：觀察1.0%利多卡因20m1腰俞穴麻醉用于混合痔手術的麻醉安全性及術后排尿情況。 方法：將89例來自成都中醫(yī)藥大學附屬醫(yī)院肛腸科病人隨機分入1.0%利多卡因20m1腰俞穴麻醉組(以下稱為試驗組)和1.5%利多卡因20m1腰俞穴麻醉組(以下稱為對照組),其中試驗組44例,對照組45例。兩組手術均采用外剝內扎硬注術,進行前瞻性的臨床觀察,并記錄兩組病例術中及術后相關的評價指標：麻醉不良反應、麻醉起效時間、麻醉療效、術中直腸牽拉反射、術后疼痛、術后首次排尿時間、術后首次排尿通暢情況、首次排尿后膀胱排空情況、術后尿潴留發(fā)生率及導尿率,應用醫(yī)學統(tǒng)計學軟件SPSS19.0對結果進行對比分析。當P0.05時表示差異無統(tǒng)計學意義,P0.05時表示差異有統(tǒng)計學意義。 結果：兩組麻醉起效時間、麻醉療效、術中直腸牽拉反射、術后疼痛(VAS評分)等方面差異無統(tǒng)計學意義(P0.05),試驗組在麻醉不良反應、術后首次排尿時間、術后首次排尿通暢情況、首次排尿后膀胱排空情況、術后尿潴留發(fā)生率及導尿率與對照組之間有顯著性差異(P0.05)。 結論：1.0%利多卡因20m1腰俞穴麻醉用于混合痔手術,具有1.5%利多卡因20m1腰俞穴麻醉相同的麻醉效果,還有術中麻醉不良反應少、術后首次排尿時間早、首次排尿后膀胱排空好、術后尿潴留發(fā)生率及導尿率低等優(yōu)點,降低了麻醉風險,減輕了術后反應。具有臨床推廣價值。
[Abstract]:Objective: to observe the anesthetic safety and urination of 1.0% lidocaine 20m1 for mixed hemorrhoids.Methods: a total of 89 patients from the anal and intestinal departments of the affiliated Hospital of Chengdu University of traditional Chinese Medicine were randomly divided into 1.0% lidocaine 20m1 Yaoshu anesthesia group (hereinafter referred to as the experimental group) and 1.5% lidocaine 20m1 Yaoshu anesthesia group (hereinafter referred to as the control group).There were 44 cases in the test group.Control group 45 cases.The patients in both groups were treated with external exfoliation and epidural injection, and the related evaluation indexes during and after operation were recorded: adverse reaction of anesthesia, onset time of anesthesia, curative effect of anesthesia, rectal reflex during operation.Postoperative pain, the first time of urination after operation, the condition of unobstructed urination for the first time after operation, the condition of bladder emptying after first urination, the incidence of postoperative urinary retention and the rate of catheterization were compared and analyzed with the medical statistics software SPSS19.0.There was no significant difference when P05 showed no statistical significance. P05 indicated that the difference was statistically significant.Results: there was no significant difference between the two groups in terms of onset time, anaesthesia effect, rectal traction reflex during operation, postoperative pain and VAS score (P 0.05). The adverse reaction of anesthesia, the first time of urination after operation, and the unobstructed first time of urination after operation were observed in the experimental group.The rate of urinary retention and catheterization after the first urination was significantly different from that of the control group (P 0.05).Conclusion 20m1 of 1.0% lidocaine has the same anesthetic effect on mixed hemorrhoid surgery with 1.5% lidocaine 20m1, less adverse reaction during operation, early urination time after operation, and bladder emptying after first urination.The low rate of urinary retention and catheterization reduces the risk of anesthesia and postoperative reaction.It has the value of clinical popularization.
